ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Responsible to ensure all potential member and members are assessed timely as the care management program dictates through effective leadership of assigned staff and process management.

  • Ensure the admissions of members/clients/patients in conjunction with leaders across various departments within Care Management.

  • Shares assessment and admissions best practices, develops and follows admission process and supervises LPN staff.

  • Works to achieve departmental goals that align with the strategic goals of the organization.

  • Directly oversees all aspects of admission process for assigned care management operational regions.

  • Ensures the adequate training of all LPNS and RNs.

  • Ensures the department coordinates new assessment appointments for face to face assessment within required timeframe.

  • Required to work with Health Care Navigation team to ensure the effective transition of patients in the CHSGa (Community Health Services of Georgia) system.

  • Participates in team meetings as required.

  • Responsible for ensuring processes meet established standards of promptness related to admissions and other quality metrics established through regulatory and internal benchmarks.

  • Ensures effective communication between Assessment Nurses, Assessment Coordinators and Health Care Navigators for Care Management Services and other associates regarding patient admissions and transitions.

  • Ensures established processes for admissions are followed and throughput of new patients occurs in a timely manner.

  • Monitors and supplies analytic data to ensure appropriate staffing to meet the needs of each region served by the Department.

  • Coordination between Departments to ensure standard of promptness are met and admissions are complete

  • Ensures that data needed from appropriate sources is obtained for nursing facility Level of Care assignment and quick throughput of the paperwork

  • Reviews weekly analytic data. Tracks info and develops action plans to address key performance indicators in conjunction with the Director of Assessments.

  • Attends and participates in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.

  • Attends and participates in mandatory in-service trainings.

  • Honors patient's rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.

  • Complies with Corporate Compliance Program.

  • Complies with all Privacy & Security programs.

  • Directs the admission process as well as arranging for transfer of patients to internal and external services.

  • Ensures contact and follow up of potential members within the standard of promptness.

  • Assists in the transition of members to system partners and outside agencies.

  • Ensure coordination and integration of the admissions process with the Care Management Operations and Quality departments.

  • Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.

  • Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.

  • Performs other duties as necessary to ensure the success of the System.

SKILLS AND ABILITIES

  • Thorough knowledge, adherence and enforcement of current regulations, as well as Care Management Services' Operational guidelines and best practices related to the operations of the waiver program and Care Management Services.

MINIMUM QUALIFICATIONS

  • RN license with a minimum of 5 years experience in the health and human services field with 5 years of supervisory experience OR

  • Bachelor's degree in Social Work or related field with a minimum of 5 years of experience in the health and human services field with at least 5 years in management or supervisory position

  • Experience in social work, home and community based services, healthcare or geriatrics preferred.

  • Experience with standard assessment tools, MDS, RAI preferred.

  • Valid Driver's License.

  • Reliable transportation.
