Job Description

DaVita is looking for a Full-Time Nurse Practitioner (NP) Manager to join our Integrated Care team.

The Nurse Practitioner Manager leads and manages our Integrated Kidney Care Nurse Practitioners in a designated market or geographical region.

In this role you will provide clinical leadership, oversight, coaching, supervision and onboarding of Nurse Practitioners to ensure that quality, cost-effective care is provided for CKD and/or ESRD patients.

You will participate in population health management and overall financial viability of the program. This role functions autonomously, yet in partnership with operational and clinical leadership on budgeting, quality initiatives, outcomes and performance measurements.

ESSENTIAL FUNCTION

Serve as the clinical lead for a team of NPs, and oversee their work. Ensure NP quality and productivity, including documenting findings, appropriate diagnoses, closing Plan of Care gaps, and accurate coding. Support NP hiring efforts and oversee onboarding of new NPs. Provide ongoing clinical guidance and training to NPs to improve understanding of CKD, ESKD, and commonly associated chronic conditions.

Serve as key leader of clinical and operational strategy working with Medical Officer and Operations leadership. Identify opportunities to streamline NP workflow and focus management intensity on high risk, high-utilization members.

Lead complex case reviews and timely support of complex case escalations of high utilization and high-risk members working with a team of specialized resources and patient's care team, and payor resources.

Know and comply with DaVita policies and procedures in accordance with State and Federal regulations for patient care and services (CMS, HIPAA, OSHA, or other governing agencies); Standards necessary to maintain organizational accreditation (such as NCQA); and, State or Federal regulations of Injury Prevention and Safety Training Program

Preferred Experience:

  • 4-6 years of NP Experience; experience in population health, Primary/family care, and/or nephrology a plus.

  • Masters degree in Nursing.

  • At least 1 year experience in supervision or leadership (did not have to be a NP at time of supervision or leadership)

  • Excellent verbal and written communication skills

  • Demonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation regarding the needs and care of chronic disease

  • Deals with confidential information and/or issues using discretion and judgment

  • Intermediate computer skills and proficiency in MS Word, Excel, and Outlook

  • Excellent organizational skills and attention to detail.

This role oversees the care of donors participating through NMDP-operated donor and collection centers. Additionally, this role supports quality assurance activities and compliance with FDA regulations, professional standards, and NMDP standards and policies. This position can be based in Minneapolis, MN or home-based anywhere in the U.S. ACCOUNTABILITIES: Donor Medical Services: * Provides clinical support to the Medical Director. * Protects donor safety, product quality, and regulatory compliance. * Oversees the care of donors participating through NMDP-operated donor and collection centers. * Reviews and interprets donor medical workup documents to determine donor eligibility and suitability for clearance. * Provides medical leadership for all aspects of donor management including donor recruitment, health screening, workup, collection, infectious disease counseling, and post-collection follow-up. * Provides consultation with network physicians and coordinators. Transplant Medical Services: * Provides medical leadership and direction for day of collection medical and logistical issues. * Manages post collection donor recovery, complications, and adverse events. * Participates in the investigation, management, and reporting of donor and recipient adverse events. * Participates in the investigation, management, and reporting of medically relevant quality incidents, including product complaints. * Provides consultation with transplant center physicians and coordinators. Quality and Regulatory Compliance: * Provides medical input into FDA and HRSA submissions including adverse event reporting and annual reports of NMDP sponsored INDs. * Ensures regulatory compliance of donor activities. REQUIRED QUALIFICATIONS: Knowledge of: * Medical/scientific knowledge of stem cell transplantation, blood banking, apheresis, and good tissue practices. * Donor medical care and management, including primary health care. * FDA Biologics, Drug and Tissue regulations (including GMP/GTP), HIPAA, and other applicable regulatory requirements. * Demonstrated program management to include ensuring quality assurance standards and strategic and operational planning. Ability to: * Provide medical and team functional leadership. * Provide problem solving and critical decision-making leadership to medical and quality staff, cross-functional teams, and with network physicians and partners. * Demonstrate leadership through tactful interpersonal skills, demonstrated ability to manage and motivate direct report staff, quality assurance standards, administration and implementation of quality improvement programs. * Meet deadlines and manage multiple tasks simultaneously. * Interpret labs, EKGs, and other routine clinical test results. * Be available during non-core business hours based on donor needs. Education and/or Experience: * Nurse Practitioner or Physician Assistant with degree or equivalent with postgraduate training in Hematology, Oncology, Stem Cell/Bone Marrow Transplantation, Transfusion or Apheresis Medicine, Internal Medicine, or Pathology. * Three years of experience in allogenic stem cell transplantation, blood banking, or apheresis.
