Ensures the highest standard of clinical care is provided to home health patients in the most cost effective manner. Monitors and analyzes service and quality indicators to develop performance improvement plans in order to meet the agency's quality and service goals.

Monitors and analyzes operational processes/reports that support cost effective and appropriate clinical practice in collaboration leadership. Serves as the branch infection control leader ensuring infection control and safety practices are maintained in the home setting. In the absence of the Branch Director, serves as the interim Branch Director.

Essential Functions

  • Participates in the hiring, orientation and ongoing supervision of clinical team members in order to provide excellent patient experiences and reduce employee turnover.

  • Supervises and supports the CSC in ensuring the provision of the highest level of patient care and service excellence.

  • Provides oversight of the clinical documentation process to ensure timely workflow processes, and supports EMR standardization.

  • Ensures completion of annual supervisory visits for all field RNs/LPNs to fulfill compliance with agency and regulatory requirements. Supports State Survey readiness and process.

  • As Infection Control leader, monitors compliance with IC policies and procedures.

  • Works collaboratively with other departments, using data from a variety of sources, in order to develop, implement and maintain initiatives to meet agency quality and service goals, particularly as it relates to hospital readmissions.

  • Leads agency hospital readmission performance improvement initiatives in order to meet specific readmission goals.

  • Ensures clinical team members receive annual competencies, annual education modules and satisfy regulatory requirements.

  • Monitors key operational metrics aligned with clinical practice through assigned administrative reports. Analyzes and identifies trends or barriers to the agency's operational and financial success. Participates in initiatives and action plans to address issues.

  • Manages, reviews and approves documentation prior to billing per agency process.

  • Approves non-formulary medical supply requests and ensures agency formulary utilized consistently and effectively. Identifies opportunities for cost savings and/or product revision based on evidence based practice.

Physical Requirements

May carry, stoop, climb or otherwise be physically active in the course of job duties. May have extended periods of sitting and/or computer work. Periodically drives to and from patient homes or to regional meetings in varied weather and traffic conditions.

Education, Experience and Certifications

Bachelor's Degree in Nursing required.

Current license to practice as a Registered Nurse or temporary license as a Registered Nurse Petitioner in the state of North Carolina and/or compact licensure in the state of residence required. 5 years Home Health experience with OASIS documentation, Medicare and Medicaid rules and regulations required.

Current Basic Life Support for Healthcare Provider status according to American Heart Association required. Must have successful experience in orienting, training, supporting new team members either as a peer preceptor or supervisor. Must have excellent presentation skills.

Must have outstanding customer service skills and a passion for patient care. Ability to effectively communicate with professionals from diverse clinical and professional disciplines. Interpersonal skills to effectively interact with all levels of internal and external customers are required.

Proficient in computer, EMR, Microsoft Office applications and other software programs necessary to perform job duties. Must maintain current state driver's license and auto insurance at all times.

Ensures appropriate, qualified and timely resource management in order to meet patient needs and agency financial, operational and quality goals. Supports service excellence and teammate engagement in daily encounters with patients and staff. Works collaboratively with the leadership to improve care and services throughout the organization and ensure operational/financial efficiency and effectiveness. Essential Functions * Responsible for daily supervision of multidisciplinary clinical team and oversight of the team's administrative support to include provision of coaching and counseling as required to ensure compliance with clinical scope of practice and standards * Provides clinical administrative support, clinical expertise to meet patient and staff needs and functions as the liaison with physician practices and hospital case management and other referral sources * Assures staffing/resource management is appropriate and assigned timely to meet patient needs and per agency policy to include documentation standards, productivity and clinical standards of practice * Analyzes the data to prioritize multi-issue situations to support appropriate actions based on the criticality of patient health and welfare, patient/staff safety and impact to effective, timely services * Identifies barriers to situation/complaint resolution (i.e. language, literacy, psychological, etc) and follows-up with the appropriate resources or leadership to address timely and effectively * Reviews and supports the development of patient specific clinical plans of care that is effectively communicated with the parties involved and supports quality patient outcomes and patient satisfaction * Supports clinical staff in the implementation of the identified plan, coordination of care delivery, and strategies to promote health and safe outcomes * Evaluates progress toward attainment of outcomes through documentation review, interdisciplinary team meetings and/or case conference * Supports billing compliance and timeliness by maintaining current knowledge of payer requirement and billing criteria, state and federal regulations, performing timely completion of CSC workflow and identifying opportunity for appropriate reimbursement * With minimal supervision, delegates elements of care to appropriate healthcare workers in accordance with any applicable legal or policy parameters or principles * Collaborates with healthcare consumer, family, and others in the conduct of clinical practice Physical Requirements Primary work is in the office environment. Requires long periods of sitting and computer work. Work may require walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be able to lift and support the weight of 50 pounds in handling patients, medical equipment, and supplies in the home/facility setting. Must speak English in good, understandable terms.
