Manager - Care Management RN

The Care Management (CM) Manager is accountable for the direction and management of the PMMC Care Management Department. This position provides leadership and oversight for the clinical care management model, staff of approximately 14 FTE, budget of approximately $1.7 M, and operations of the PPMC CM department.

Required Qualifications:

• Bachelor's Degree in Nursing.

• Oregon Registered Nurse License upon hire.

• National Provider BLS - American Heart Association upon hire.

• 5 years of progressive supervisory/management experience including program planning, performance management, staff development and evaluation.

• 3 years of Care Management experience.

Preferred Qualifications:

• Master's Degree in Nursing.
• 2 years of relevant social services utilization management and/or quality management.
