Summary

Under general direction, responsible for performing administrative professional work coordinating projects and activities for single-family rehabilitation programs. This position will also coordinate projects between divisions, departments, public agencies, and private sector representatives. May exercise supervision over assigned staff.

Essential Job Functions:

  • Monitors and evaluates assigned programs and projects, sets deadlines, analyzes issues, and provides recommendations.

  • Implements, monitors and evaluates new and existing programs.

  • Develops and maintains reporting tools including programmatic budgets, commitments and expenditures.

  • Reviews and prepares invoicing packets fiscal for processing.

  • Develops and maintains client information systems.

  • May assist in the preparation and monitoring of the annual budget.

  • Prepares written reports and presentations on project progress and results.

  • Works to ensure coordination between divisions within the department.

  • Organizes and prepares written summaries of project reports and action plans in a timely manner.

  • Performs related duties and fulfills responsibilities as required.

Job Requirements:

  • Bachelor's Degree from an accredited college or university with preferable coursework in Business, Public Administration, or a related field.

PREFERRED QUALIFICATIONS:

  • Two years of experience working with SAP.

  • Two years of experience with client management systems.

  • Two years of experience with Federal CDBG and HOME Grant Administration.

  • Two years of experience with process analysis and process improvements.

  • Advanced Microsoft Office Excel skills.

Responsible for assessment, transition of care, and care coordination for children and families identified as meeting program criteria. Conducts research, analyzes complex statistical data, develops planning timetables, evaluates systems policies and procedures, and makes recommendations to achieve goals and objectives. May exercise supervision over assigned staff. Essential Job Functions * Implements actionable individual and family care plans inclusive of: support for self-management, education and engagement of individual and family, coaching and counseling of individual and family, and cross-sectional communication and collaboration with healthcare providers, clinical office personnel, and school personnel. * Analyzes complex data, policies, practices, systems, procedures, and develops written summaries on findings and recommendations, including ensuring consistent, documented follow-up of appropriate referrals. * Evaluates actionable individual and family care plans per defined program measures and reports findings per program requirements; ensures development and implementation of interventions appropriate for underserved pediatric populations and their caregivers; * Uses evidence-based techniques to educate and engage the home-based asthma management program team and supervises team. * Tracks interventions and case management outcomes through ongoing data collection (i.e., Quality of Life Questionnaire, pre-/post-tests, chart reviews, database) and prepares program progress reports. * Uses communication skills to effectively engage community members either as a member of an inter-professional team or an interdisciplinary partnership. * Creates and prepares reports, presentations, and materials about projects, including asthma health information, and communicates information to multiple audiences including the public, community partners, City management, and health professionals. * Acquires and maintains current knowledge of asthma management. * Prepares, monitors, and oversees the budget and inventory of equipment and supplies, especially medical supplies; ensures supplies are ordered in a timely manner. * Follows all rules and laws protecting Protected Health Information (PHI) associated with the Health Insurance Portability and Accountability Act (HIPAA) as well as other regulatory agencies. * Evaluates and recommends staffing levels to ensure effective utilization of human resources. * Consults with departments or divisions to improve productivity and delivery of services. * Performs related duties and fulfills responsibilities as required. Job Requirements * Bachelor's Degree from an accredited college or university with major coursework in Business, Public Administration, Public Health, Healthcare, or a related field. Master's Degree preferred. * Five (5) years of experience in a relevant field, including but not limited to management, case management, analysis, or policy development. PREFERRED QUALIFICATIONS: * Valid Class "C" Texas Driver's License. * Current Texas License to practice as a health professional (e.g. nurse, respiratory therapist, social work, paramedic, etc.) * Ability to obtain Asthma Educator Certification (AE-C) within one (1) year of employment. * Three (3) years of supervisory experience. * Three (3) years of case management experience. * Bilingual in English and Spanish. * Available to work flexible hours, including some early evenings and/or weekends.
