Schedule: 8:30 am- 5:00 pm EST, Monday- Friday


Overview: The Community Connector's primary focus is to conduct outbound and inbound calls to Managed Care Organization members. This role involves tracking members’ health conditions monthly and reporting to the Case Manager. The Community Connector assists members in staying safe and happy within their community, uploads documents, and supports Case Managers with necessary tasks.

  • Fluency in Spanish

  • Good typing speed
  • Good listening skills with empathy
  • Ability to follow directions
  • Self-motivated
  • Ability to complete projects and tasks by due dates

Day-to-Day Responsibilities:

  • Receiving inbound calls from the queue for 2 hours per day
  • Making 300 outbound calls per month
  • Attempting 35-40 calls per day
  • Completing 15-18 calls per day

Required Years of Experience:

  • At least 1 year of relevant experience

Required Licensure / Education:

  • High school diploma or equivalent

System Access Requirements:

  • Access to CCA, Salesforce, QNXT, HHAX, Genesys, HCS system, QCSS system, and E-paces
