The LPN Home Care Nurse provides skilled nursing care to patients and families in their homes, which supports the total person. Works collaboratively within an interdisciplinary team (RN, Home Care Aide, Social Worker, P.T., P.T.A., O.T., C.O.T.A., S.L.P., and office support staff) to coordinate patient care utilizing community resources as needed. Participates in staff education. Works under the general direction of a Registered Nurse.
Provides superior customer service to internal and external clients, customers and patients.
Assesses patient health status including demonstration of functional status, records and related health data
Accurately, promptly and thoroughly documents patient care, observations, interventions and evaluations
Participates in analyzing and interpreting data from assessment; makes informed judgments in conjunction with the RN
Provides appropriate care for a range of physician-ordered treatments (blood sugar, pulse oximetry, wound care, IVs, phlebotomy, telehealth, etc.)
Must possess current Mass. LPN license.
Must start the process of obtaining NH license upon acceptance of offer of employment if working in the Methuen, Waltham and Ayer Branch or a RI license if working in the Fall River Branch.
Must possess current valid driver's license and a car.
Travel by car to patient's homes within assigned territory.
Must have demonstrated competence in community health or med/surg patient care setting, providing direct patient care for a minimum of one year.
