Seton Medical Center has a long tradition of providing patient-centered, quality care. Founded in 1893, the 357-bed medical center now serves 1.5 million residents of San Francisco and northern San Mateo County with comprehensive inpatient and outpatient medical specialties, as well as emergency and urgent care services. Its sister facility, Seton Coastside, is a 116-bed skilled nursing complex offering inpatient care and the only 24-hour standby Emergency Department on the Pacific Coast between Daly City and Santa Cruz. The Licensed Vocational Nurse (LVN) provides appropriate nursing care under the direction of the Registered Nurse (RN) in compliance with the standards of care at Seton Medical Center and Seton Medical Center

  • Coastside and the LVN Nurse Practice Act; demonstrates respect for the individual and maintains the dignity of the patient and family unit.


  • Care for all types of patients and perform those skills and procedures taught in an accredited program of vocational nursing. Perform all care within the scope of practice for vocational nurses and as practices at Seton Medical Center.

  • Appropriately assess and document the patient's initial condition and assesses and documents ongoing changes. Participate in patient interventions, plans for care and evaluation of care.

  • Plan and initiate patient teaching as related to the patient's illness. Includes teaching and psychosocial interventions with both patients and family.

  • Participate and document discharge planning for the patient on initial admission and throughout hospital stay.

  • Participate in unit based quality assurance activities.

  • Administer and monitor the effects of medications as prescribed by a physician and in accordance with practice at Seton Medical Center.

  • Perform venipuncture following the standards of practice at Seton Medical Center.

  • Demonstrate safety-conscious behavior consistent with the Safety Program.

  • Demonstrate ability to document information in the patient record accurately and in a timely, concise manner.

  • Demonstrate participation in professional self-development.

  • Use effective oral and written skills to communicate with other members of the health care team as well as patients and their families.

  • Show appropriate leadership skills while directing other members of the health care team.

  • Perform other appropriate duties as assigned by a Registered Nurse.



  • Graduation from an accredited School of Licensed Vocational Nursing

  • Current California licensure as a Licensed Vocational Nurse.

  • Current BLS certification through the American Heart Association for the Healthcare Provider

  • Senior Licensed Vocational Nurses must have successfully completed courses in Intravenous Therapy approved by the State Board, and Pharmacology as recognized by the Board of Vocational Nurses and other unit specific certifications. All Licensed Vocational Nurses must complete IV certification within six (6) months of hire.

Work Experience:

  • Previous experience preferred.

  • Able to understand and follow all policies, procedures, guidelines, rules and regulations of Nursing Service, Seton Medical Center and Seton Medical Center

VIEW JOBS 2/28/2019 12:00:00 AM 2019-05-29T00:00 Licensed Vocational Nurse (LVN)/ Patient Health Coaches are unlicensed health professionals and are primarily responsible for assisting patients throughout the routine visit and helping their assigned panel with health management. LVN/ Patient Health Coach will work collaboratively with other health care members to provide comprehensive, quality and accessible health care services to patients under the supervision of a physician, nurse practitioner, registered nurse team leader and nurse manager. Critical activities include: patient education, help patient to develop and achieve self-management goals, close the loop, medication reconciliation, and population management. ESSENTIAL JOB FUNCTIONS: * Assists in the provision of clinical services such as, but not limited to, taking vital signs, performing EKG, immunization, skin test, blood draw, insertion of urinary catheter, and collects and prepares specimens as necessary. * Operates various medical equipment, e.g., electrocardiogram, audiometry, bilimeter, glucometer, and autoclave. * Performs venipuncture and skin puncture. * Administers medications and vaccines within his/her scope of practice. Refer intravenous infusion to a Registered Nurse. * Assists providers during physical examination, treatment, and procedures. * Trains new nursing staff and MA/LVN externs. * Knows the scope of practice for his/her own profession and that of the Medical Assistants (MAs), and assigns tasks accordingly. * Supervises MAs. * Makes appropriate referrals to other departments within the agency as well as to outside agencies. * Refers to a Registered Nurse for any telephone or drop-in triaging cases. * Provides patient health coaching to patients with chronic medical problems. * Checks completeness of provider's work. * Educates patients about health problems, treatment regime, and medical procedures. * Implements medication plan demonstrating knowledge of drug interactions and side effects. * Performs maintenance of equipment, and stocks of supplies for exam rooms. * Performs in-office procedures as outlined in the medical clinic and nursing manual. * Completes forms required for different programs, e.g., pharmacy, authorization, and referral forms. * Work some Saturdays, on a rotational basis. * Responsible to tract and follow up on patients with continuing health care needs such as, but not limited to, health screening, breast, cervical, prostate, glucose, cholesterol, colorectal etc. * Performs other job duties as required by manager/supervisor. ESSENTIAL JOB FUNCTIONS AS A HEALTH COACH * Provides basic health education on diet and exercise in a 1-on-1 setting or group classes. * Performs medication reconciliation with patients and discusses medication adherence strategies. * Assist patient in action planning and formulating chronic disease self-management goals. * Follows-up by phone with patients one week after visit with PCP to make sure patient understands any medication changes. * Communicates with patient's PCP to ensure care coordination. * Track and document patient goals in EHR. * Performs other job duties as required by manager/supervisor. QUALIFICATIONS: * Graduate of an accredited Licensed Vocational Nursing Program. * Current California LVN license. * Ability to physically assist a patient, manages patient flow, and room patients. * Knowledge of basic computer skills. Type at least 35 w.p.m. * Recent experience in a clinic or outpatient setting preferred. * Pediatric experience an asset. * Current documentation of Basic Life Support is required. LANGUAGE: * Must be able to fluently speak, read and write English. * Fluent in Chinese (Cantonese and/or Mandarin), Vietnamese, Spanish and other languages may be required depending on location. * Fluency in other languages are an asset.
