Full policy is provided upon requestQualifications Ability to work independently with minimal supervisionDemonstrates good verbal and written communication, and organization skillsCompassionate and caring demeanorAbility to remain calm in stressful situations Current, active, license as an RN or LVN by the California Board of Vocational Nurse and Psychiatric Technician ExaminersCPR certifiedValid driver’s license, auto insurance, reliable transportation and willingness to drive to patient locationsPreferred one year experience in hospice and palliative care or similar fieldResponsibilitiesBe responsible for maintaining patient information and patient health consistent with the mission and goals of the AgencyProvide intermittent visits or shifts of skilled nursing care, performs prescribed medical treatments, including pain management and symptom control, provides nursing observations, provides teaching regarding the expected course of the disease, care techniques and other health measures to the patient, family or other primary care giver, provides counseling and supportive care to the patient and family, and participates in the care plan development and implementationPerform procedures at a level of care standard within the medical community, scope of practice consistent with the Vocational Nursing Practice Act and follows Agency's current policies and proceduresProvide accurate and timely documentation of patient services according to the plan of careContinuous Care as directedCommunicate with the interdisciplinary team regarding changes in the patient's condition and/or needsField nurse required to make visits to patients during on-call shift within the service areaEnsure quality and safe delivery of hospice care nursing services
