Responsibilities
The Quality Improvement Coordinator will be responsible for identifying quality gaps, risk adjustment gaps and
outreach needs for members managed within Medicare advantage plans as well as the UTHET population at
large. Individuals within this job classification are accountable for contributing to the assessment,
implementation, and documentation of quality activities in accordance with UTHET standards to deliver efficient,
effective, and quality patient care. If you have a clinical licensure, you will have additional responsibilities such as
initiating standing orders, using clinical judgement, provider patient education as needed.
Performs as a team member to support and enhance the system-wide quality improvement program. Provides positive staff support and education and acts as a resource to clinical staff and administration. Provides support and assists in the implementation of tools, guidelines, and best practices to improve efficiency, quality of care, and patient outcomes. Promotes and supports the implementation and monitoring of system policies and guidelines, and other state and federal laws or regulations.
The Quality Improvement Coordinator will work with a multi-disciplinary healthcare team to assist in facilitation of meeting quality metrics, risk adjustment activities and other assignments for a designated panel of managed care patients. Shall perform as a team member to support and enhance the system-wide quality improvement program. Responsible for collecting and organizing data to build and maintain patient registries to identify gaps in care and lead to better healthcare outcomes for patients. Expected to lead the employed and embedded QI care partners in the collection of quality data, audits and conduct outreach as necessary.
Will be proficient in use of EMR, payer platforms and population management tools. Will be knowledgeable of workplace health and safety guidelines (CDC), physician office practice, preventive health maintenance, national evidence-based guidelines, patient safety, and case management. Promotes and supports the implementation and monitoring of system policies and guidelines and other state and federal laws or regulations.
Qualifications
Job Requirements:
High school diploma
One to three minimum years clinical or healthcare experience
Preferred Job Requirements:
LVN, preferred
Quality experience, preferred
Epic EMR, preferred
Medicare Advantage experience, preferred
Microsoft Excel knowledge, preferred
