Job Description

Assist with PPO and HMO membership to improve health care quality and outcomes for the customer and the client. Encourage customers to engage in their preventive health care and focus on healthy living.

Required Skills

Assist the Network Operations team with Stars Rating goals, Cigna Collaborative Accountable Care goals, and other miscellaneous projects. Familiar with HEDIS specifications and criteria.

Able to understand and connect with the customer. Able to place phone calls to customers regarding their health care and schedule physician office visits. Familiar with a queue line and Outlook email. Able to use Excel and keep organized call tracking logs.

Required Experience

Minimum Requirements:
Clinical License Required LVN

Preferred Requirements:
Managed care experience, ideally with Medicare or Medicaid HMO.

Regulatory compliance experience: CMS, HEDIS, STARS. Knowledge of Microsoft Office products. Spanish Speaking.

The Field Clinical LVN participates in planning, providing, and evaluating patient care at the appropriate skill level. They have a duty to provide a standard of care that meets or exceeds that of a reasonably competent and knowledgeable Licensed Practical Nurse. The LVN performs duties within their scope of practice delegated by a physician or ARNP. The LVN will report to the PSU Clinical Team Manager. The successful candidate organizes the clinical environment in patient care situations. Support includes but is not limited to assisting the ARNP, including those skills listed under Job Functions below along with various other procedures as directed by the ARNP. The LVN completes FOC screenings and documents those results on the encounter form. The LVN should communicate data to the ARNP or other specified members of the clinical team within the provider's office as needed. The LVN delivers quality customer service. Ensures policy and procedures are followed including infection control, privacy and confidentiality. This position performs all duties within the scope of an LVN, including Focus on Care screenings, phlebotomy, and collection of vital signs. Primary Responsibilities: * Reviews the patient history in ePRG for contraindications to FOC screening procedures * Rooms patients according to company standards * Performs all nursing care duties within the scope of a Licensed Vocational Nurse, including DPN testing, Quantaflo testing, spirometry testing, administration of the PHQ9, DRE examinations, phlebotomy, administration of FOBT/FIT tests, scheduling mammograms or colonoscopies as appropriate, and schedules patient return visit to provider's office * Organizes, cleans and stocks assigned FOC equipment and lab supplies as needed to bring with him/her to all field visits * Communicates patient concerns, inability to collect FOC tests, and observed patient issues to the WellMed ARNP if present, or the delegated clinical representative in the provider's office at the time of the patient screening * Responsible for completing FOC skills assessments of providers as needed * Responsible for completing all patient packets and testing procedures in a timely manner and ensuring that those results are submitted to the proper person within the PSU and a copy is left with the delegate at the provider's office * Performs all other related duties as assigned This position requires travel in the Houston area to clinics and provider offices. This position is based out of the office in NW Houston (I-10 Beltway area) and will require time spent in the office preparing paperwork and you will also have to transport medical equipment to these clinics. Hours are Monday - Friday 8a to 5p and could potentially have to work occasional Saturdays as well. Required Qualifications: * High school graduate or GED equivalent * Current unrestricted Texas LVN license. * 2 years of related LVN clinical experience * Working knowledge of medical terminology and ICD-9 and CPT coding * Computer literacy required with ability to complete basic EXCEL spreadsheets * Must possess and maintain a current driver's license and operating vehicle within the applicable state to provide transportation of equipment and self to and from various provider's offices * Ability to react calmly and effectively in emergency situations * Good communication and customer service skills both in person and on the phone Preferred Qualifications: * 3 years of LVN experience in a medical setting Physical & Mental Requirements: * Ability to lift up to 50 pounds * Ability to push or pull heavy objects using up to 300 pounds of force * Ability to stand for extended periods of time * Ability to use fine motor skills to operate equipment and/or machinery * Ability to properly drive and operate a company vehicle * Ability to receive and comprehend instructions verbally and/or in writing * Ability to use logical reasoning for simple and complex problem solving * Ability to travel in a vehicle for extended periods of time * Occasionally requires exposure to communicable diseases or bodily fluids Careers with WellMed.
