Position: Licensed Vocational Nurse (LVN)

Reports to: Director Patient Care Services

Job Description/Requirements:

Home healthcare nurses provide continuous nursing care to patients in medically fragile conditions. At AccentCare, our nurses spend quality 1:1 time building a relationship with each of their patients. We are committed to service and quality that shows in our compassion and in positive patient outcomes.

As an LVN you will:

  • Provide nursing shift care in accordance with the written plan of treatment signed by the physician and all subsequent orders that authorize and direct changes

  • Provide (on a daily basis) paperwork as required by agency to maintain accurate and complete medical records.

  • Conduct and pursues the business of agency in accordance with sound nursing and medical practices and represents agency in a manner calculated to instill confidence in patients, physicians and the general public.

  • Perform nursing services only under the guidance and supervision of a registered nurse.

Requirements of Qualified Candidates:

  • Graduate of accredited practical/vocational nursing program

  • Currently Licensed Practical/Vocational Nurse in state(s) of agency operation

  • Must possess and maintain valid CPR certification while employed in a clinical role

  • One (1) year LVN or RN experience (must be paid nursing experience)

Bonus Info: $500 after 30 days of employment and another $500 after 90 days of employment. Employee (LVN) must work an average of 32 or more hours during that timeframe (First 30 days - 137 hours and 90 days 411 hours). *Must have 1 year of LVN experience to qualify for sign on bonus.

VIEW JOBS 7/15/2020 12:00:00 AM 2020-10-13T00:00 Overview * SIGN-ON BONUS* The Field Nurse (LPN/LVN) conducts in-home/field-based visits to address urgent clinical issues with Landmark patients. While on-site, the Field Nurse will evaluate patients and communicate, via telephone or secure video, with the supervising Landmark provider. The Landmark provider will direct the evaluation and care provided. All activities will fall within the employee's scope of practice as set by the State they are practicing in. * Assigned Shift - Saturday & Sunday 9am - 6pm Responsibilities * Patient assessment and evaluation: Vital signs, physical exam, point of care testing (E.g. - glucose levels), venipuncture, EKG, medication compliance and reconciliation, adherence to discharge instructions, etc.• Patient treatment and procedures: IV therapy, nebulizer treatments, wound care, medication administration, etc. Treatment directives are only per the supervising Landmark Provider's direction.• Patient education: Common chronic conditions and education topics include congestive heart failure, COPD, diabetes, hypertension, fall risk assessment, overall health literacy per the direction of the Landmark Provider.• Landmark Provider collaboration: Discuss evaluation and treatment with supervising Landmark Provider. This collaboration will occur during every patient visit unless the Field Nurse is following an approved Landmark protocol (e.g. Post-Hospital Visit).• Care coordination: Schedule follow-up telephonic or in-home interactions as needed.• Documentation: Assist supervising Landmark Provider with documenting findings and outcomes of the visit.• Other duties as assigned. Qualifications * Requires a current LPN or LVN license and a high school diploma or equivalent• Ability to work a variety of shifts.• Unrestricted driver's license and reliable personal vehicle and automobile insurance.• Urgent care, emergency room, or community home health experience preferred.• Comfortable working with electronic medical records and mobile technology.• Solution-oriented mindset and work approach. Charisma and a natural ability to connect with others.• Excellent communication and interpersonal skills, with a strong emphasis on empathy and building rapport with patients.• 1+ years of experience evaluating, assessing and providing patient care. * Phlebotomy and IV experience is preferred.
