Under the general direction of a qualified registered nurse , the Licensed Vocational Nurse assists in the nursing process by instructing patients and care givers in self care and providing basic nursing care as needed.

Primary Job Responsibilities:

Provides direct nursing care according to the medical plan of treatment and nursing care plan

Utilizes physical and psychosocial assessment skills, performs multi-system assessments and identifies current and potential care needs

Carries out physician orders for treatment and visit frequency per plan of treatment

Organizes and completes patient treatment and tasks per established care plan

Implements and updates care plan in collaboration with the Clinical Manager and Primary Care Clinician to reflect progress toward goals and outcomes

Coordination care among team members and documentation in the clinical record per policy

Educates and trains patients and caregiver on exercises and equipment needed to aid recover

Provides education and instruction specific to patient and caregiver assessed needs, abilities and readiness

Accurately documents ongoing assessments, clinical findings, skilled care and plan for next visit according to the established care plan

Must be flexible with schedule including, but not limited to, the ability to participate in on call and/or department rotation for night and weekend coverage

Education

Graduate from an accredited vocational school of nursing or equivalent.

Licensures and Certifications

Licensed Vocational Nurse

  • LVN

Current California Blood Withdrawal certification, or to be obtained within 30 days of hire date OR Certified Phlebotomy Technician Level I - CPT I

Basic Life Support Certification

  • BLS

Automobile Insurance

  • INSURANCE

Driver License, Valid and in State

  • DL NUMBER

Experience

Minimum 1 year of professional experience required.

Direct patient care experience within the past five years required.

Home health or community health experience preferred.

Skills and Knowledge

Basic competencies in the fundamentals of nursing and pharmacology are required.

Demonstrated skills in effective written and verbal communication.

Ability to recognize the needs and concerns of diverse groups of people.

Ability to maintain harmonious constructive working relationships with internal and external customers (patients, families, members of the interdisciplinary team, clerical and management staff, physicians and community resources).

Ability to use and give instruction of adaptive equipment.

Must be able to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work, think critically to both effectively plan and organize department operations consistent with Sutter Care at Home's strategic and operating objectives and to effectively solve unique problems as they arise or identify when to consult supervisor.

Must be able to deal with challenging work environment with time demands and occasional conflicting priorities.

VIEW JOBS 10/31/2018 12:00:00 AM 2019-01-29T00:00 Under the supervision of the Clinical Manager, the Physical Therapist is responsible for the delivery of appropriate, adequate, effective, efficient direct patient care services assigned by the Primary Care Clinician and approved by the Clinical Manager. Responsibilities include implementation of the physician plan of care, obtaining physician orders, supervision of home health aides, and coordination of care within an interdisciplinary team and participating in effective utilization management and control of patient supplies. Consistently provides care based on the age-related needs of the unique population with consideration of their development/psychosocial needs, environmental safety, educational needs, communication style, and level of understanding. The Physical Therapist is accountable to the Primary Care Clinician for ensuring that the plan of care is implemented following professional standards of care and agency specific protocols. Education Graduate from an accredited physical therapy program. Masters in Physical Therapy or DPT preferred. Licensure Current California PT license; valid California driver's license; current auto insurance; current BCLS/CPR certification. Experience: One year of professional experience in an acute or similar outpatient setting required. One year of experience in a home health and/or hospice position preferred. Direct patient care experience within the past five years required. Knowledge Exhibits excellent oral and written communication skills: with legible handwriting for documentation of patient records in a clear and concise manner. Basic understanding of home care services, clinical standards of care and practice, Title XXII, Medicare Conditions of Participation, CHAP home health accreditation requirements, QI outcomes/benchmarking, infection control; good understanding of reimbursement patterns for Medicare and other payors; good working understanding of budget and other financial reports. Special Skills/Equipment Self-starter with a high degree of initiative, motivation, flexibility, energy and creativity. Ability to form harmonious working relationships with internal and external customers. Demonstrated leadership and negotiation skills and the ability to manage resources for patient care in a cost effective manner. Excellent collaborative and problem solving skills with customers and management. Ability to exercise independent sound judgment in planning and providing patient care. Demonstrated ability in physical, psychosocial, and environmental assessment skills and in implementing plan of treatment. Excellent organizational skills. Must be able to meet company productivity standards as indicated by organization and manager. Must be able to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work, think critically to both effectively plan and organize department operations consistent with Sutter Care at Home's strategic and operating objectives and to effectively solve unique problems as they arise or identify when to consult supervisor. Must be able to deal with challenging work environment with time demands and occasional conflicting priorities. Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements. Is responsible for maintaining all required licensure and certifications.
