We're looking for someone to join our team as a LVN, Care Management Coordinator (Home Health) who:

Coordinates incoming office referrals and verifies insurance coverage. Coordinates incoming calls from patients, family, physicians, and disciplines. Ensures that all personnel are informed of patient's hospitalization and follow-up with patients admitted to other hospitals. Coordinates small program(s) with limited budget/impact. Applies substantial knowledge and experience to perform a wide range of advanced activities and/or determines how to use resources to meet schedules and goals; serves as working supervisor for team or work group.

Essential Functions:

  • Handles and coordinates incoming calls related to patients, physicians, referrals and agency services regarding physician orders, patient questions and patient referrals.

  • Facilitates telephone communications between, and demonstrates effective communication skills, with nursing, therapy, aide, social services, physicians, etc., regarding changes in patient/staff schedule, test results, etc.

  • Assists with staffing/scheduling activities, soliciting input from managers.

  • Provides follow-up for hospitalized patients, ensuring subsequent agency referral.

  • Performs other job-related duties as assigned.

You'll be successful with the following qualifications:

Education:

  • High School Education/GED or equivalent: Preferred

  • Associate's/Technical Degree or equivalent combination of education/related experience: Preferred

Work Experience:

  • Experience in a healthcare setting: Preferred

Required Licenses/Certifications:

  • Licensed Vocational Nurse (LVN) licensure in the state of practice: Required

We're looking for someone to join our team as a Sr. Customer Care Associate (Home Health) who: Provides day-to-day administrative support for the Home Health patients and department. Essential Functions: * Handles incoming calls from plan members and healthcare providers for the assigned health plans. Provides information as needed for questions related to medical, dental, vision, and prescription drug claims based upon the benefit descriptions and plan provisions of a variety of group plans. Documents all calls. * Receives referrals and pre-certification requests from providers as applicable. Reviews referral/pre-certifications for proper benefits and eligibility. Enters referrals and pre-certifications data into the computer system. Reviews referrals and medical record information as applicable to facilitate claims department with claims adjudication. Researches and documents rejected or erroneous claims and forwards them for correction as needed. * Performs other job-related duties as assigned.
