LVN

Overview

Demonstrates knowledge and understanding of normal limits of vital signs and commonly used lab tests by recognizing abnormal findings and reports them to the RN. Under leadership of RN the LVN gathers data on patient's physical and emotional needs. LVN gathers data on family psychological needs. Performs simple procedures skillfully (e.g. dressing changes, urinary catheterizations, tube irrigations). Participates in patient care plan conferences and in discharge planning.

Responsibilities

Sets priorities according to patient care needs

Completes initial assessments & re-assessments of patient care needs, based on hart review and/or direct observation.

Documents care on patients according to diagnosis and established standards of practice.

Reassess as patient condition warrants and prepares transfer and discharge as appropriate.

Recognizes signs and symptoms of potential emergencies to include behavioral and physical changes. Initiates appropriate actions and timely interventions.

Recognizes indicators and proper reporting of abuse.

Utilizes appropriate methods to access risk and prevention patient falls & skin break down on intact skin.

Assessment/alternative for management of restraints.

Performs and documents legibly/timely and accurate nursing interventions to meet patient care needs.

Coordinates total nursing care to the patient using the plan of care to provide continuity of care.

Assumes the responsibility for teaching and documenting patient education appropriate and consistently.

Assures complete follow-up instructions are given with demonstration or verbalized understanding by the patient prior to discharge and documented in the medical record.

Qualifications

Current CA licensure as a Licensed Vocational Nurse required. Current Basic Life Support certification. Current ACLS Certification. IV certification.
