Position: Long Term Services & Supports Community Partner (LTSS CP) Program Director
Supervisor Chief Operating Officer/VP Clinical Services
In collaboration with the Northeast Independent Living Program (NILP) LTSS CP Program Director, the Elder Services of the Merrimack Valley (ESMV) LTSS CP Program Director is responsible for the implementation, management, supervision, and evaluation of the LTSS CP Program. The Program Director will provide supervision to ESMV LTSS CP staff. The Director will build and enhance relationships with Community-based Organizations (CBOs) and Accountable Care Organizations (ACOs) in collaboration with ESMV Leadership.
Essential Functions: (Core duties or tasks that are fundamental, and not marginal to the performance of the job)
Provide leadership and management for the LTSS CP Program.
Oversight of the day to day operations of the LTSS CP Program.
Provides upward communication to the Chief Operating Officer/VP Clinical Services for supervision, direction and support.
Serve as a liaison to ACOs, CBOs, provider partners and other organizations for the LTSS CP Program.
Ensure program operates in compliance with contractual obligations, service delivery standards and adheres to agency policies and procedures.
Oversight of LTSS CP budget and staffing requirements.
Provide internal and external resources to the Care Navigator teams as needed.
Oversee program quality assurance and compliance with contractual reporting.
Utilize data to monitor service delivery and ensure that desired outcomes are being met.
Provide leadership, direction, and motivation for staff on the importance of continuous quality improvement efforts and interdisciplinary team development.
Manage employee relations and community relations as they pertain to the LTSS CP program.
Assure coordination of the LTSS CP program with all programs and services sponsored by the agency.
Participate as a member of the agency's Senior Management Team as appropriate.
Participate in the development and implementation of the agency's strategic plan.
MA/MS in Health Care Administration, Public Health Policy, Human Services, Social Work, Nursing or related field.
A minimum of five years-related managerial experience in a supervisory capacity.
Applicable knowledge and experience in healthcare, managed care and human services continuums.
Highly organized with strong management skills, able to evaluate and control complex delivery systems.
