LTC Services Screener I- Screens requests for Community Care Services Eligibility programs under Health and Human Services regarding applicant eligibility and process for unit assignment. Screens and provides general and specific information about Long Term Care services to applicants, consumers and general public.
Makes appropriate referrals to other program areas and community partnerships in the Region. Assigned as needed to a remote community partners site at least twice a week. Provides extensive coordination and problem solving.
Works with limited supervision and has considerable latitude for the use of initiative and independent judgement. Essential Job Functions:
Answers the toll-free intake line for Region 10 and Region 01, Communicates on a basic level with others (internally and externally) to provide, exchange or verify information. Provide inquiries, address issues, or resolve/report problems or complaints.
Routes calls and takes messages as appropriate. Operates a computer using the Service Authorization System Online (SASO) and Texas Integrated Eligibility Redesign System (TIERS) to document, update or inquire into case record. Interviews and assesses applicants for potential eligibility for Community Care Services Eligibility program under Health and Human Services.
Serves as client advocate and agency representative in locating available resources and making referrals to community resources as deemed appropriate. Be able to work a regular and predictable work schedule and be willing to report to a remote community partner site. Completes additional tasks, projects or other assignments upon request of management staff. Knowledge Skills Abilities:
Valid Driver's license. Initial Selection Criteria:
Experience working in a fast-paced, high-volume, high stress environment and with limited supervision. Knowledge and/or experience in reference to Community Social Services/Programs.
Experience and knowledge indicating understanding of the skills and abilities needed to do the job to include but not all inclusive; Experience in screening and referring customers/applicants for benefits. Experience in gathering and verifying information. Experience in interviewing and screening consumers for services.
Experience interpreting and applying policy and/or procedures. Minimum of three years of experience with customer service and data entry while on the telephone or in a call center. Application indicating bilingual; English/Spanish preferred.
Knowledge in the usage of IT systems for example; Excel, E-mail, Word, and Internet. Application showing ability to complete data entry at minimum of 50 wpm. Application showing willingness to travel at least 25%. Additional Information:
Must be able to work a regular and predicable work schedule in accordance with agency leave policy. This position is NOT eligible for flex-time and/or compressed work week schedules.
This position is included in a career track series. Job offer will be at the entry level, Screener I salary $2075.83 to $3,047.58.
