Overview1. Assists physician, advanced practice providers and nursing staff in the provision of high quality, comprehensive patient care in the ambulatory setting, appropriate to the age and needs of the patient population served.

Observes, records and reports to the medical provider, symptoms, reaction and changes that may be indicative of physical and/or psychological conditions of the patient. Verifies immunization status of pediatric patients.

Prepares patients for diagnostic and/or treatment procedures. Reviews patient's chart and diagnostic tests to facilitate appropriate follow-up care with the provider. 2. Assists physician, advanced practice providers and nursing staff in obtaining patient data, communicating with other office staff and physicians, patients, families, and a variety of health care agencies to ensure continuity of patient care, and appropriate documentation.

Assists the medical provider with minor medical and /or surgical procedures performed with the office setting. C ommunicates with and act as a liaison between the patient, their family, medical providers, office personnel and other community agencies Obtains and documents appropriately history, physical assessment data and other pertinent information relating to the patient 3.

Assists physician, advanced practice providers and nursing staff in the provision of patient education through application of teaching-learning principles appropriate to the age and needs of the patient population served by the office, to help patients prevent and/or effectively cope with medical problems, and to promote a healthy lifestyle. Outstanding Very Good Good Fair Poor N/A Acts as a resource person to help clarify and educate patients and family members to their diseases, treatment protocols, prescribed therapeutic interventions and follow-up instructions. Acts as a liaison for patients for the purpose of providing or promoting lifestyle modifications by making appropriate referrals to various departments or outside community agencies for educational opportunities.

Provides health education to the patient in person or by telephone in accordance with office protocols and under the direction of the provider 4. Assists in the maintenance of a therapeutic environment that promotes effective and cost efficient patient care, and that provides for the safety of patients, staff, and visitors. C oordinates individual efforts with those of other members of the health care team to ensure quality care and patient satisfaction as well as a professional and congenial work environment for the office staff and physicians Outstanding Very Good Good Fair Poor N/A Maintains appropriate supplies in the exam and treatment rooms, as well as the nursing station, in a cost-effective manner.

Actively participates in maintaining a clean, safe, therapeutic environment, applying principles and policies related to infection control and safety Participates as a member of the health care team striving to promote a comfortable and safe environment for patients. Demonstrates a commitment to respecting and maintaining patients' rights and confidentiality of patient information. Assists in the development and implementation plan to correct problems and improve care and increase patient satisfaction on an ongoing basis. Participates in special projects assigned by the office manager/practice administrator as the need arises or a problem is identified
