LPN's in this role will support the Care Coordination Telemedicine Team and must be competent with communication and technological skills. Telemedicine triage nurses function as a primary point of contact to respond to patient email inquiries related to a variety of health conditions. LPN's support with assessment, planning, and care coordination.
This is a telephonic position, working in an office-based environment, located in Secaucus, NJ.
The core hours for this position are 2pm-10pm Eastern Time M-F and alternating Weekends.
Identify, assess, plan, coordinate, and monitor activities to address the patient's medical needs
Consult with and refer to appropriate healthcare professional (ie. PCP/ Specialist, RN) accordingly
Prevent, reduce, and resolve potential or identified problems by adhering to nursing protocols and processes
Support the prior authorization process
Have the ability to recognize when telemedicine approaches are not appropriate and follow protocols to escalate and triage emergent referrals
Must consistently meet established productivity, schedule adherence, and quality standards while maintaining good attendance.
Provide quality, consumer focused customer service.
Accurately document consumer interaction and appropriately follow all processes and procedures.
Stay abreast of all customer specific details, to ensure high quality customer service.
Ability to function in a fast paced environment
Demonstrate established customer service behaviors and standards

LPN license in the state of NJ
2 years of clinical experience
Proficient Computer Skills (Email and Microsoft Word)
Basic knowledge of disease prevention
Experience with Medicaid, Medicare, and other commercial health payers
Knowledge of medical terminology
Ability to work rotational overnight and weekend shifts
