Monday through Thursday 11 a.m. to 7:00 p.m. Friday 9:00 a.m. to 5:00 p.m. About the Role:

The LPN will perform telephonic patient outreach and is responsible for interviewing patients about their medications related to prescriptions and over-the-counter items. Topics of discussion include adherence, adverse effects, current health status, and therapeutic recommendations. No Hands-on Care.

Key Responsibilities: Maintains current knowledge of medications. Perform telephonic medication reviews.

Accurately document clinical data in patient charts. Identify potential issues based upon information gathered during the interview with the patient. Follow all established CSS Health policies and procedures including but not limited to HIPAA, privacy and security policies and procedures.

Operate office equipment such as voice mail messaging systems, email, and use of various software applications to support operational processes. Maintain appropriate training levels for duties and responsibilities described. Perform other duties or tasks, as assigned, or required.

Must have high-speed internet. Qualifications: Graduation from an accredited nursing program (LPN) with an active license. At least 1 year of experience in the healthcare industry is preferred.

Prior call center experience preferred. Ability to work as a team member, be professional and have telephonic communication skills, have attention to detail, and exercise professional work ethics. Understanding of Medicare and Medicaid programs preferred.

Understanding of HIPAA, Privacy, Safety, and Compliance guidelines. Knowledge of MTM and Transition of Care procedures and processes preferred. Intermediate understanding and ability to navigate a database system.

Intermediate understanding of Microsoft Office Suite (Excel, Word, etc.) and knowledge of efficient record-keeping techniques.
