Duties

Lake Forest School District 67 is seeking a Licensed Practial Nurse (LPN) certified Teacher Assistant at Cherokee Elementary School for the 2024-2025 school year.

Description

The Licensed Practical Nurse will assist students with various duties including medical services, classroom instruction, supervision of student(s) (e.g. bus, lunch, playground, assemblies, etc.) and other duties assigned by the building administrator to support students in the least restrictive environment across all educational settings. The position is responsible for assisting with toileting, feeding including g tube feeding, administering appropriate medications, tending to first aid needs, responding to emergencies, providing health education, and performing other related work as required. The position works under the direct supervision of an RN and reports major activities to the building administrator. The LPN will serve as a teaching assistant when not assigned to support health services.

Essential Functions

  • Provides case management of students with chronic health conditions including identifies, monitors, and manages students with chronic disease and disabilities, and documents the services provided in accordance with orders and care plan.

  • Implements Emergency Action Plans written by medical professionals (i.e., FARE Plans, Asthma Action Plans, Diabetic Care Plans, Seizure Action Plans).

  • Accurately documents student health needs in PowerSchool.

  • Conducts physical healthcare assessments and screenings, including but not limited to temperature checks, diabetic sugar levels, signs of aspiration, etc.

  • Assists with maintaining school health records to assure compliance with state mandates including care plans, immunizations, child health examinations, and school dental and eye examinations.

  • Participates in building level meetings, weekly CII meetings, and problem solving meetings

  • Provides emergency crisis nursing care for ill and injured students and staff.

  • Consults with parents, teachers and medical professionals in collaboration with the building nurse

  • Supports daily clean up and wiping down surfaces to uphold sanitation standards as required for a safe school environment.

  • Administers medication as prescribed by a physician, reports adverse reactions or contraindications to the parent/guardian, building nurse, physician, Principal, and District level nurse

  • Assists the student with using adaptive equipment such as wheelchairs (manual, electric), Hoyer lift, specialized walkers or chairs, respiratory devices, communication devices, feeding devices, and others depending on student needs.

  • Subs in Health Offices throughout district 67 as a last resort.

  • Assists with feeding, toileting and monitors fluid/food intake and output for assigned students.

  • Transfers patients safely according to training from the physical therapist.

  • Provides in a timely fashion requested documentation of specialized student services in accordance with School Board criteria established for the purposes of the Medicaid

  • Provides counseling and guidance to students, parents and staff regarding health-related attendance problems.

  • Communicates health information with the necessary stakeholders as appropriate.

Qualifications

Education, Experience, and Licensure

  • IL Paraprofessional or Substitute Teacher License (required), PEL (preferred)
  • Licensed Practical Nurse license (required)
  • Certification in CPR/AED (required)

  • Possesses the proven ability to work harmoniously and effectively with teachers, students, parents, and district administrators in accomplishing the purposes of the position.

Salary/Benefits

  • Starting Range - $25-$30/Hour

  • Calendar

  • 10 Month (178 work days at 7.5 hours/day from mid-August through the 1st week of June)

Additional Notes

Physical Demands & Work Environment

While performing the duties of this job, the employee is regularly required to be mobile and transport instructional materials from one setting to another within the building. The employee must occasionally lift and/or move up to 50 pounds.
