PURPOSE OF YOUR JOB DESCRIPTION:
The Licensed Practical Nurse (LPN) is a licensed nurse who participates in the plan of care to provide competent, compassionate and individualized nursing care to patients and their families as developed by a registered nurse or nurse practitioner. The LPN also participates in the medical plan of care under the supervision of the physician or nurse practitioner and in activities to facilitate safe and efficient practice operations.
DELEGATION OF AUTHORITY:
Reports to the Provider and to the Clinical Manager of Physician Practices.

PERFORMANCE REQUIREMENT / DUTIES AND RESPONSIBILITIES
Participates in the plan of nursing care as developed by, and under the supervision of, a registered nurse or nurse practitioner.
1.Provides and maintains a therapeutic environment for patients and families.
2.Communicates the plan of care and other pertinent information to / from other team members.
3.Provides nursing care appropriate to age group of patients consistent with the practice scope of service.
4.Observes, reports, and documents pertinent patient information, in accordance with the standards of documentation for ambulatory care nursing.
5.Performs nursing procedures within the scope of practice of an LPN and as outlined in the Nursing Procedure manual.
6.Collects data to assist the registered nurse or nurse practitioner to evaluate outcomes of nursing care for patients and to modify the plan of care as appropriate.
7.Works with the registered nurse or nurse practitioner to define standards of excellence for patient care.
8.Participates in and contributes to performance improvement activities.
Assures efficient patient flow through the office
1.Assist with the patient intake process:
Assures patient record complete prior to visit.
Escorts patient to exam room and checks identification.
Obtains information about chief complaint.
Reviews current medications and prescriptions needing refill.
Obtains vital signs, age-appropriate measurements and other measurements as applicable.
Reviews record for immunization needs.
Documents all of the above.
2.Instructs / assists patient to prepare for examination by provider including setting up special equipment as needed.
3.Notifies provider when patient is ready and communicates pertinent information obtained during the intake process.
4.Assists patient through check-out process when exam is complete.
5.Cleans and prepares room for next patient.
6.Responds to needs to expedite visits and keep on schedule.
Participates in the medical plan of care under the direction of the physician or nurse practitioner:
1.Assists provider with patient examination, procedures, and treatments within the parameters defined by the Department of Nursing.
2.Administers medications within the parameters defined by the Department of Nursing after successful orientation.
3.Collects & tests specimens and instructs patient about specimen collection at home, as applicable.
4.Performs other tests, procedures, and treatments within the LPN scope of practice.
5.Performs designated tasks to track test results and communicate results to patients.
6.Receives patient messages, reviews with provider, and takes action as directed.
7.Faxes, mails or telephones in prescription renewals as directed by provider.
8.Assists provider in coordinating care with / making referrals to community agencies.
9.Collaborates with other professional and non-professional personnel in maintaining recognized standards.
Participates in orientation of new permanent and temporary staff members:
Teaches and directs other clinical support staff for whom she / he is responsible.
Appropriately utilizes MGH nursing resources.
Appropriately utilizes equipment and supplies.
Provides input to supervisor regarding practice needs.
Participates in practice and other committees for formulation of practice policies & procedures.
Collaborates with registered nurse or nurse practitioner to identify / meet own learning needs.
Participates in relevant continuing education programs.
Adheres to regulatory requirements for annual training.
Previous LPN experience
Knowledge of EMR
Medical Office experience a plus
