Job Summary

Performs routine nursing care of the ill, injured or infirm under the direction of a licensed professional (RN) nurse, nurse practitioner and/or a physician. Demonstrates competence in caring for patients with moderate to complex health issues. Participates in the planning, implementation and evaluation of nursing care in accordance with the scope of the LPN as defined by the State Board of Nursing and health system standards. Complies with the administrative policies and clinical protocols as outlined in policy and procedure manual. Also supports operations through administrative duties as required. Administratively responsible to the Site/Business Manager and the Ambulatory Nursing Director for nursing practice issues.

Job Responsibilities

Practice

  • Participates in following the Nursing Process to deliver safe care: planning, implementation and evaluation under the direction of the Registered Nurse.

  • Participates in patient data collection and physical measurements and communicates findings with care team.

  • Collaborates with family, team members and other caregivers to update plan of care, patient/family education and other needs as directed by the RN and/or Physician.

  • Participates in the care of complex patients in collaboration with the RN and/or Physician.

  • Recognizes unexpected patient outcomes and notifies RN and/or Physician for revision to plan of care.

  • Performs various lab studies as instructed by the Provider and in compliance with POCT guidelines.

  • Participates in telephone intake within their scope of practice.

  • Knowledgeable in Emergency Preparedness and maintain office in state of preparedness.

  • Role models appearance and behaviors that promote a positive image.

  • Interacts with parents/patients and colleagues with respect and dignity.

  • Spends the time with families listening for avenues to improve on the ideal patient experience.

  • Arrives at work on time and is ready to start the shift and predicts challenges and obstacles.

  • Provides an accurate report on the patient and family to members of the team.

  • Actively participates in patient care and makes recommendations to the plan of care.

  • Introduces self to patient and family stating both name and title.

  • Participates in site, departmental, and organizational meetings to meet institutional goals.

  • Reads nursing articles, synthesizes information and initiates staff discussion(s).

  • Employs current knowledge, incorporates evidenced based nursing practices and applies critical thinking, interpersonal and technical skills necessary to complete their assigned job responsibilities within their scope of practice.

  • Assists with research studies being conducted.

  • Able to identify own learning needs. Seeks out opportunities to meet needs and attends all required programs.

  • Applies knowledge from continuing education to enhance personal knowledge and skills.

  • Communicates and utilizes patient education materials relevant to patient population.

  • Monitors own usage of supplies and equipment. Identifies opportunities for improvement and makes recommendations.

  • Follows and is knowledgeable of Health System's standards on medication and immunization management.

Required Licenses, Certifications, Registrations

BLS - Basic Life Support

LPN / Temp Practice

Required Education and Experience

Required Education: Graduation from an NLNAC and/or CCNE accredited licensed practical nursing program.

Required Experience: Two years' experience.

Preferred Education, Experience & Cert/Lic

Preferred Experience: Pediatric experience.

Additional Technical Requirements

Computer Competency

  • Basic use of a computer (mouse, keyboard, printer, USB ports).

  • Basic use of Microsoft Office (Word processing, spreadsheets, presentations and Skype).

  • Web browsing, intranet search, document access.

  • Use of shared network file space.

  • Basic use of patient facing technology. (e.g. IV pumps, point of care devices, CR monitors).

  • Navigation and use of clinical communication systems.

Information Literacy

  • Determines the nature and extent of clinical information needed and uses the appropriate technology to access it.

  • Accesses needed information effectively and efficiently.

  • Evaluates information and its sources critically and appropriately incorporates it into clinical practice.

  • Evaluates outcomes of the use of information.

Information Management Literacy

  • Navigation of the electronic health record system.

  • Ability to locate and review specific patient data in various clinical information systems.

  • Effectively uses clinical decision support tools.

  • Understands policies and procedures, confidentiality, security, and privacy regulations regarding the use of clinical information systems.

  • Ability to collect clinical data using quality improvement tools.

Primary Responsibilities: The Registered Nurse position is a multi-functional role that includes triage telephone duties as well as direct patient care duties. Telephone Triage: The Telephone Triage role serves as the point of contact for incoming calls which includes screening, documenting, and prioritizing calls , transferring messages appropriately to and from patients/providers, and providing pertinent information to patients. These duties include but are not limited to: 1. Answers calls "live" coming into the telephone triage nurse unit from Nemours patients and families. 2. Ascertains reason for call and quickly escalates to the physician when appropriate. 3. Schedules appointments according to the patient's needs. 4. Monitors Audix voice mailbox, retrieves information, and enters data into electronic medical record. 5. Ensures documentation of incoming calls is complete and accurate to ensure timely call back by nurses. 6. Performs routine filing and faxing of documents; updates and distributes information on physician on-call schedules. Direct Patient Care Duties: The Registered Nurse provides nursing care to patients in the primary care pediatrics practices as directed by providers. The position is responsible for patient flow and assisting providers as necessary with examination, procedures, and administration of immunizations which are adapted to the patient's development stage. These duties include but are not limited to: 1. Performs patient assessments. 2. Performs and documents patient intake (height, weight, head circumference, blood pressure, vision and hearing screening, reason for visit, chief complaint). 3. Assists providers with procedures, including but not limited to, suturing, suture removal, sterile procedures, minor surgical procedures, gynecological exams, and catheterizations. 4. Performs phlebotomy, via venipuncture or capillary techniques, to obtain blood specimens. Transcribe, collect, label and arrange transport for lab specimens. Performs back office laboratory tests. 5. Administers immunizations, oral and/or injectable medications. Administers PPD and interpret results. 6. Orders and stocks appropriate inventory of medical-surgical supplies for lab and exam rooms. 7. Provides age and developmental appropriate care to infants, children, adolescents and adults. 8. Monitors patient flow including preparing patients and rooms for physicals and sick visits.
