Full-time LPN position in Family Practice Clinic

Hours: Monday through Friday 8a-4:30p; One day per week 12p-7p; Hours may vary


Under the direction of Site Physician(s) and Practice Manager, responsible for assessment and treatment of patients in an outpatient/office setting. Provides treatment and follow-up education consistent with patient's age and cultural needs. Responds to all patient inquiries, prioritizing responses and routing urgent situations to a physician, PA or NP (hereafter Provider). Maintains accurate and appropriate documentation and follows patient privacy in accordance with HIPPA and DMG policies and procedures. Acts as role model for all other care givers in the coordination of patient care. Performs to the level of a licensed practical nurse as dictated by the Illinois Nurse Practice Act and DuPage Medical Group.


  • Consults and coordinates with health care team members to assess, plan, implement, and evaluate patient care. Assists Provider in taking patient histories, obtaining vital signs, and preparing patient for routine exams or procedures, including collecting and processing specimens. Observes, records and reports the patient's condition and reaction to treatments and/or drugs. Where appropriate, administers first aid, immunizations and injections; assists Provider with minor surgical procedures, including the application and/or removal of casts, posterior molds and splints, sutures and staples. Administers prescribed medications and treatments in accordance with approved nursing techniques and as directed by the Provider; including oral, cutaneous (i.e., patch), intramuscular, and subcutaneous and IV routes (RNs only). Prepares and orders tests: fills out lab requisitions, test orders, patient forms and referral forms as per the Provider's request. Tracks and obtains all test results for Provider review; notifies patients of results as directed. Assesses the patient's physical, age specific, and psycho-social needs as related to the disease process, treatment, and education.

  • Maintains accurate and detailed patient records, charts and other pertinent information; posts tests and examination results. Ensures medical chart/lab test results are available and complete and all records are appropriately secured in the patient chart.

  • Delivers patient education, according to the individualized needs of the patient and as prescribed by the Provider and/or Group policy. Educates patient and/or family members about diagnostic procedures, medications, nutrition and maintenance of health and wellness. Responds to patient questions and appropriately provides reassurance while responding to patient concerns.

  • Responds to/refers incoming patient treatment-related phone calls. Triages patient messages and responds in accordance with departmental policy and procedures. May assist in arranging tests or admissions, calling in prescriptions, etc. Reviews and prioritizes same day messages. Under direction of the Provider, RNs can assess and determine patient needs... LPNs can convey symptoms to a Registered Nurse, physician, PA or NA for best course of action.

  • Responsible for routine maintenance and preparation of medical equipment; cleans, disinfects, and/or sterilizes instruments and equipment following CDC safety and OSHA standards. Follows Group policy and OSHA guidelines for handling of blood and body fluids and uses PPE appropriately. Maintains quality assurance records of the disinfection process; serves as a role model of safe office practice.

  • Performs and delegates a variety of tasks to facilitate patient office visits and procedures. Aware of coding and reimbursement issues in delivering care. Available to other office clinical and non-clinical staff to answer questions and provide assistance as appropriate. Assists in orienting and training new staff. Seeks opportunities to improve or enhance nursing and leadership skills.

  • Ensures narcotics are secured and appropriately accounted for on a daily basis. Inventories drugs and relays or orders additional supplies. Ensures appropriate transport of expired medications to the centralized mail room.

  • May schedule tests and procedures and obtain insurance pre-authorization.

  • Performs all other duties/responsibilities as assigned.

  • Graduation from an accredited Licensed Practical Nursing program.

  • Knowledge of nursing skills in accordance with nursing standards in the Nurse Practice Act.

  • Must have a current CPR certification and a current and valid Illinois Nurse License.


  • Prefer a minimum of one year of professional nursing experience in a clinical or hospital setting but may consider new grad under specific circumstances.

About the Unaccompanied Childrens Shelter The Unaccompanied Childrens Shelter is home to adolescent girls and boys through the Office of Refugee Resettlement (ORR). This program provides a safe setting for children during their first days in America until a permanent setting can be established for them. POSITION SUMMARY: The Nurses primary responsibility is to assure the provision of quality health services for Maryville youth according to the standards and guidelines of the Office of Refugee Resettlement (ORR). The employees primary duty is work that requires advanced knowledge in the field of nursing. Position requires the consistent exercise of discretion and judgment. ESSENTIAL FUNCTIONS: * Is knowledgeable of and follows Maryville Academy Standard Medical Operating Procedures; * Establishes positive working relationships with the Treatment Team, program staff, and other Maryville nurses; * Develops and maintains lines of communication with outside contractors and consumers including ORR, physicians, dentists, therapists, and others as needed; * Monitors the health care delivery system at assigned sites, to include: * Medical clearance system; * Medication administration, storage, documentation and consents; * Delivery of prescribed patient care; * Patient medical record system; * Physicians' orders, prescription and lab work implementation in an accurate and timely manner; * Implements any home adaptations, as needed; * Oversee the medical equipment providers, as needed; * Ensures that medical information is up to date; * Coordinates with the program management staff in the discharge planning system; * Develops and maintains a system to provide medical consultation to case managers, Maryville staff and contracted staff; * Develops and provides medical workshops, groups, and CPR training for staff and clients; * Supervises the infection control program and quality improvement and safety program as it pertains to medical issues; * Communicates with the therapists, dietician(s), and psychiatrist(s) and all members of the multi-disciplinary team; * Participates in treatment team meetings, management team meetings and quarterly review staffing as needed; * Schedules client medical appointments as necessary; * Attends client medical appointments as indicated for the health and safety concerns of each client; * On-call 24 hours as needed, for crisis situations; * Participates in continued performance quality improvement; * Performs other work related duties as assigned. MINIMUM REQUIREMENTS: Education/Experience: * Minimum of Licensed Practical Nurse. Licensed Registered Nurse, preferred; * Experience in pediatric nursing; * Physical assessment course work preferred. Skills /Knowledge: * Bilingual English/Spanish language competency required; * Ability to communicate effectively, verbally and in writing; * Basic computer proficiency including working knowledge of MS Word, Excel, and PowerPoint; * Current certification in infant/child/adult CPR; Physical Demands: * Required to frequently sit, stand, bend, stoop, walk and drive for extended periods; * Required to walk up and down stairs; * Required to occasionally lift/move objects and/or youth weighing in excess of 40 pounds; * Must possess the ability to provide physical restraint for students threatening to harm themselves or others; * Must have the visual, hearing, and learning capabilities sufficient to perform the essential functions defined above. Environment and Scheduling: * Work is performed in designated clinical and program settings; * Duties may involve contact with youth who may be threatening, aggressive and/or under the influence of drugs or alcohol; * Duties involve contact with students who may be traumatized; * Must be flexible and available to work additional hours when necessary.
