Under the supervision of the supervising Pharmacist, the Medication History Technician (MHT) will perform medication histories through patient/family/significant other interviews for patients arriving via the Emergency Department to be admitted to the inpatient units. The technician will utilize External Rx History and Cerner PowerChart Medication by History documentation to enter the patient's current medication list into the electronic medical record. The process involves the following steps; 1. interview of patient/family member/significant other to collect and verify medication history, 2. review of prescription history via electronic claims database,3.contacting outside pharmacy/PBMs as necessary, 4.compiling all information gained in steps 1 through 3 to create an accurate medication history, and 5. accurate and timely electronic medication history.
1.Obtains and clarifies patient's home medication list by utilizing drug information resources, contacting community pharmacies, interviewing patient/ family/significant other and reviewing prescription bottles brought to the Emergency Department by the patient to ensure proper identification of doses and drugs including prescription medications, over the counter drugs and herbals/supplements. Returns prescription bottles to the patient's family/significant other after obtaining information to assure return home for use as directed after discharge.
2.Responds to nurses, physicians, pharmacists or other requestors and appropriately manages, triages, or ensures follow-up in a timely manner regarding medication history information and other related information.
3.Troubleshoots electronic hardware and software applications as needed and appropriately notifies IT of any malfunction needing higher attention so as to minimize down times, delays in medication reconciliation information input and disruption in electronic communications/processes.
4.Obtains the patient's allergies and communicate with Emergency Dept Staff to improve documentation in Cerner PowerChart, including symptoms, reaction, and serverity.
5.Directs, triages or communicates to the appropriate caregivers or staff all questions or inquiries from patients or families that are outside the scope of taking the medication history.
6.Identifies and participates in ongoing quality assurance and improvement processes to ensure an appropriate historical medication list is obtained.
7.Assists with evaluation of data on compliance of medication reconciliation processes for admitted patients and makes recommendations to improve the process.
8.Fulfills all compliance responsibilities related to the position.
9.Performs other duties as assigned.
Required: The role requires an appropriate level of understanding and training in medical terminology and medication related terms.
Current/fluent pharmacy terminiology and dosages, ability to interview patient's for accurate and complete medication history, prroficient with computer, typing, and organization. Positively interacts with patients, other hospital guests, and hospital staff to enhance the quality of service provided. Professional and discreet. Required to concentrate for extended periods of time, provide high output of work on short notice, establish priorities, make quick accurate decisions.
Minimum Experience: one year
Desired: Pharmacy Technician preferred.
