OverviewCatholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.* 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends * Great Healthcare benefits, and education reimbursement of $7,000 annually * Additional compensation for visits over your caseloadJob DetailsOur Home Care LPN's: * Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients. * Measure vital signs, Administer IVs, change dressings, clean wounds as needed * Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations. * Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.*

Provide patient and family education for disease management and health promotion s directed by physician's plan of treatment.Minimum Qualifications: * New York State Licensed Practical Nurse (LPN) license * Minimum of one year of recent hands on experience * Basic Life Support CertificationSalary RangeUSD $32.00 - USD $40.00 /Hr.The pay for this position ranges from $32 - 40 per hour. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience.
