The LPN School Health provides school health services as directed by the School Health Services, Clinical Coordinator (Registered Nurse) to the student population serviced. Responsibilities:
1.

Provides school health services as directed by Registered Nurse and contracts. 2. Maintains appropriate student health records and documents services rendered. 3. Provides the clinical care necessary within the department, including routine, special needs and emergency first aide. 4.

Communicates with parents/guardians, school authorities, and allied professionals. 5. Participates in the educational efforts of the department and the hospital. 6. Demonstrates the knowledge and skills necessary to provide care for the physical, psychosocial, educational, and safety needs of the patients served regardless of age and developmental status. 7.

Other duties as required. Other information: Technical Expertise 1.

Experience as an LPN is required. 2. Experience in a school setting is preferred. 3. Minimum 6 months experience in healthcare is required. 4.

Proficiency in MS Office [Outlook, Excel, Word] or similar software is required. Education and Experience 1.

Education: Graduate from an accredited school of practical nursing is required. 2.

Certification: Licensed Practical Nurse licensure is required. 3. Years of relevant experience: 6 months is required. 4. Years of experience supervising:

Essential Functions Provides services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team. May not alter the plan of care. As assigned, visits patient and provides nursing care according to plan of care. Participates in the in-service program according to LHC policy. Completes a progress note for each visit, and submits progress notes to the agency daily. Participates in the QA/QI plan and process. Continually observes and reports the patients nursing needs. Instructs the patients family in the care of the patient and maintenance of a healthy environment for the patient. Observes, records, and reports to the supervising nurse and/or physician the patient's response to treatment and changes inthe patient's condition. Adheres to and participates in the Care Management process. Participates in staff conference regarding patient care. Education & Experience License Requirements Current LPN licensure in state of practice. Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
