Job Summary: Under supervision of the Program Manager, provides nursing care and services for residents.

Works collaborative with other nursing staff to coordinate and provide direction of care during weekday operations. Serves as coordinator of care for scheduled weekend and holiday shifts. Ensures care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care, the Program for all-inclusive Care for the Elderly and the State of Wisconsin. Includes CBRF regulations for any Coordinators working at the CBRF.

Location: 1555 South Layton Blvd., Milwaukee, WI 53215

Hours: 20 hours per week; AM/1st shift, 6:30AM - 3:00PM. Every other weekend and every other holiday per year.

Education: Diploma from accredited program in practical nursing required.

Experience: 2 years experience in a health care setting serving the elderly and medication passing experience required. Experience in a long- term care or CBRF setting preferred.

Education

License/Certification: A current Wisconsin LPN license and current CPR/AED certification are required.

VIEW JOBS 5/22/2018 12:00:00 AM 2018-08-20T00:00        Ascension At Home is looking for a Coordinator of Quality Care for our Wisconsin market POSITION SUMMARY: This position provides comprehensive planning, implementation, and evaluation of the Quality Assurance and Process Improvement (QAPI) and clinical compliance activities. The Quality Care Coordinator serves as an internal expert for a wide range of activities including patient/family satisfaction, clinical benchmarking data, infection surveillance, referral source satisfaction, and other quality initiatives. In addition, the Quality Care Coordinator will coordinate a comprehensive record review program. This position collaborates with service line area leaders to maximize utilization of quality data. Essential Duties and Responsibilities: * In collaboration with clinical leadership will identify quality assurance and process improvement activities and priorities for the HH and/or HO service line * Assures completion and submission of quality reports and projects including: * Quality Outcome Report * Patient incident/complaint trending * Clinical outcome data as needed (include but not limited to: infection control data, falls reduction program) * Audit and Appeals data * Maintain knowledge base of quality improvement, conditions of participation and home care regulations. * Working knowledge of statistical and data analysis * Coordinates all regulatory driven audit and appeal requests for home health and/or hospice service line with Compliance department. * Coordinates all requests for additional record reviews. * Coordinate a comprehensive program to assure appropriate risk management and compliance activities that promote an environment of compliance and safety. * Identification of potential high risk compliance issues * Participate in the development of appropriate process and policy to manage risk * Assure processes and policies are in alignment with all applicable regulations and standards * Monitor patient incidents/complaints via the appropriate software, evaluating trends escalating areas of risk * Assure appropriate structure of a quality assurance/process improvement program * Provide level appropriate analysis of clinical, quality, and patient experience outcomes. * Assist agency leadership in the development of improvement plans * Assure evaluation (verification/validation) is a key component of all improvement plans * Adhere to all company policies and procedures * Adherence to and compliance with information systems security is everyone's responsibility. It is the responsibility of every computer user to: Know and follow Information Systems security policies and procedures. Attend Information Systems security training, when offered. Report information systems security problems. Education/Licensing/Certification: * Nurse or Therapist with a Master's Degree or equivalent. Candidates without a master's degree but with extensive related work experience may be considered * Current licensure Experience: * A minimum of three (3) to five (5) years of recent home care experience. * Minimum of 1 year experience in quality improvement required Knowledge and Skills: * Computer literacy * Working knowledge of business office solutions (Word, Excel, etc.), with keyboarding skills sufficient for efficient performance of these activities * High level of problem-solving ability and analytical skills. * Ability to work collaboratively in large quality initiatives while being independent in core functions. * Working knowledge and understanding of process and system improvement. * Well-developed communication and interpersonal skills. * Ability to interact with customers in a supportive and user-friendly manner. * Ability to perform a variety of duties characterized by frequent change. * Familiarity with operational and clinical activities in-home care is preferred.
