Cleveland Clinic's Lodi Hospital is seeking a Licensed Practical Nurse (LPN) to add to their dedicated and supportive team of caregivers. As an LPN, you will work under the supervision of a physician, Registered Nurse or Office Manager to provide top-quality nursing care to patients in the Primary Care department. An LPN in this role is familiar with obtaining vital signs, preparing exam rooms, administering medication, scheduling, answering telephones and assisting physicians during treatments, exams and tests.

Responsibilities:

  • Prepares and maintains exam/treatment rooms and equipment following infection control guidelines.

  • Readies patients for examination and treatment and instructs patients during the collection of samples and tests.

  • Assists practitioner during examination, treatment, and testing of patients.

  • Acts as the liaison between the patient and practitioner regarding test results, medication changes, patient conditions, reactions to drugs/treatments, patient education, and practitioner recommendations.

  • Assures accurate and complete documentation of patient histories and vital signs.

  • Performs lab tests, procedures, vaccinations, and treatments per the practitioner's standing orders.

  • Coordinates all aspects of prescriptions including contacting pharmacies and patients and obtaining prior authorizations for medications per insurance requirements.

  • Answers the telephone promptly and courteously, screens calls, takes messages, provides information and follows up on patient issues in a timely manner.

  • Collaborates with peers for Medication Management including ordering and stocking medical supplies and vaccinations/immunizations, completing the Sample Medication Log, and properly disposing of out dated medications.

  • Completes quarterly Performance Improvement reports and performs chart review.

  • Completes mandatory lab proficiency testing to maintain Clinical Laboratory Improvement Amendments (CLIA) certification.

  • Maintains and reviews patient records, charts, and other pertinent information.

  • Coordinates Patient Assistance programs for free medications between pharmaceutical companies and patients who meet the established criteria.

Education:

  • Graduate of an accredited school of practical nursing.

Certifications:

  • Licensed Practical Nurse currently licensed in State of Ohio.

  • Maintains current BLS certification.

Complexity of Work:

  • Knowledge of the principal and skills viewed to provide patient care and treatment.

  • Knowledge of organizational policies, regulations and procedures to administer patient care.

  • Establishes and maintains effective working relationships with patients, office staff and the public.

  • Knowledge of Electronic Medical Records preferred. Excellent customer service skills.

Work Experience:

  • One year of professional nursing experience in a physician practice setting preferred.

Personal Protective Equipment:

  • Follows Standard Precautions using personal protective equipment as required.
