Licensed Practical Nurses are critical members of the PreferCare staff and provide in-home clinical care services to patients, ranging from preventive care to managing acute/chronic conditions, focusing on individuals with complex medical conditions who are at high risk for hospitalization or emergency department visits. Field Clinicians will be the main point of contact for their attributed patients and help connect them to the PreferCare programs, community-based services, and other supportive programs. Field Clinicians conduct in-home patient visits and facilitate in-home telehealth encounters with PreferCare nurse practitioners to assess patients and help document all aspects of care, including care plans and summary notes for the clinical record. This role requires working independently and collaborating with nurse practitioners and physicians on the PreferCare team and with our partnering medical practices. This role is ideal for a Licensed Practical Nurse (LPN) or a Paramedic/Medic with extensive experience in providing patient care.

ESSENTIAL JOB DUTIES:

To perform this job, an individual must accomplish each essential function satisfactorily, with or without a reasonable accommodation.

  • Conduct comprehensive in-home assessments of patients' health status, including physical, emotional, and social aspects.

  • Develop and implement individualized care plans in collaboration with a multidisciplinary healthcare team.

  • Administer medications and other basic treatments as prescribed.

  • Monitor patients' conditions and report any changes to the remote Nurse Practitioner or other healthcare providers.

  • Provide education and support to patients and their families on managing chronic conditions and preventing complications.

  • Utilize telehealth technology to coordinate care and consult with the remote Nurse Practitioner.

  • Document patient visits and care provided in accordance with organizational policies and regulatory requirements.

  • Ensure continuity of care by coordinating with other healthcare providers and community resources.

  • Participate in quality improvement initiatives and continuing education activities.

  • Other duties as assigned

JOB REQUIREMENTS:

Required Computer Software/Equipment Used:

QUALIFICATIONS:

  • Licensed Practical Nurse (LPN) or certified Paramedic/Medic with a valid North Carolina license.

  • Minimum of 2 years of clinical experience, preferably in home health, emergency medicine, or a related field.

  • Strong assessment and clinical skills with the ability to manage complex medical conditions.

  • Proficient in using telehealth technology and electronic health records (EHR).

  • Excellent communication and interpersonal skills, with the ability to build rapport with patients and their families.

  • Ability to work independently and as part of a multidisciplinary team.

  • Valid driver's license and reliable transportation.

  • Experience in managing chronic diseases such as diabetes, heart failure, COPD, and other complex conditions preferred

  • Certification in Basic Life Support (BLS) preferred

  • Familiarity with local community resources and healthcare systems preferred

SUPERVISORY RESPONSIBILITIES:

  • Does not have supervisory responsibilities

  • Supervisor frequently determines priorities

  • Occasionally needs manager's direction due to extraordinary circumstances

WORKING CONDITIONS:

  • Audio-Visual: Good

  • Hearing: Good

  • Ability to lift to 40 pounds

  • Ability to lift and move patients as needed.

  • Frequent travel to patient homes within the service area.

  • Flexibility to work various shifts, including evenings and weekends as needed.

  • Pathogens: Risk of exposure to bloodborne pathogens and other contagious illnesses

  • Subject to standing, walking, sitting, bending, reaching, kneeling, pushing and pulling

  • Work is typically performed in a well-lit; comfortable temperature-controlled environment

  • Above-average conversational noise is frequent

  • Position may require flexible hours, unscheduled overtime or occasional week-end work

  • Must be able to speak and write in English

  • Travel may be required
