Job Summary: Under the supervision of the Clinic Supervisor, provides nursing care and other services supportive to the provision of care for members. including frail or elderly population and/or a developmentally or physical disabled population with diverse ethnic and cultural backgrounds. As a member of the interdisciplinary team the Licensed Practical Nurse (LPN) works with members to increase independence and community integration through a model of self-determination which maximizes physical, mental, and functional health.
Location: Provides coverage for the following Community Care sites
Hours: Hours of work can vary from site to site and include day shift, pm shift, and night shift.
The level I position requires a minimum of 2 shifts per month, 1 on which is a weekend day. If there is no weekend shift available, 2 weekday shifts will be required. This position also requires 1 holiday per year.
The level II position requires a minimum of 4 shifts per month, 2 on weekend shifts. If there is no weekend shift available, 4 weekday shifts will be required. This position also requires 2 holiday shifts per year.
Education: Diploma from accredited program in practical nursing required.
Experience: A minimum total of one year recent experience with older adults or persons with developmental and/or physical disabilities in a hospital, homecare or nursing home setting is required. One year of practical nursing experience is preferred.
License/Certification: A current Wisconsin LPN license and current CPR/AED certification are required.
