Position Overview

Responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.

Essential Job Functions

  • Responsible for the delivery and coordination of quality patient care in compliance with physician orders.

  • Continuously observes and assesses patient condition and care needs and reports changes in condition to the supervisor and/or physician as appropriate.

  • Documents all activities, assessments, nursing actions, responses and coordination of care in a timely manner whenever care is delivered.

  • Participate, implement and update the nursing care plan.

  • Takes appropriate nursing action based on assessment and achieves expected outcomes.

  • Recognizes changes in patient needs and responses requiring intervention and implements care to prevent risk or reduce risk.

  • Accepts responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies and the Nurse Practice Act.

  • Provide care utilizing infection control measures that protect both the staff and the patient according to OSHA standards.

  • Educates the patient and family regarding the disease process, self-care techniques, and prevention strategies, and in meeting the patient's nursing needs.

  • Maintain knowledge of competencies related to the nursing profession by participating in educational programs, continued education units, internal learning management skills and skill evaluations.

Requirements

  • Graduate of an accredited school of nursing.

  • Current, unrestricted state license as a Licensed Nurse in the state of practice

  • Current CPR certification

  • Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures

Additional state specific requirements:

  • South Carolina One (1) year of pediatrics experience

  • California One (1) year of experience required working under current nursing license

  • Louisiana One (1) year of experience required working as a licensed nurse

  • Continuing Education as required by state

Preferences

  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting

  • Home health experience

Reports To: Clinic Manager or Clinic Area Supervisor Hours: 40 Hours (Monday-Friday) Essential Job Responsibilities: * Demonstrates the clinical knowledge and judgment required to assist in oversight of assigned clinic * Works directly with Clinic Manager or Clinic Area Supervisor to orient the schedule, and coordination of staff duties * Fulfills patient care responsibilities as assigned that may include checking schedules and organizing patient flow; accompanying patients to exam/procedure room; assisting patients as needed with walking transfers, dressing, collecting specimens, preparing for exam, etc.; collecting patient history; performing screenings per provider guidelines; assisting physicians/nurses with various procedures; charting; relaying instructions to patients/families; answering calls, and providing pertinent information * Fulfills clerical responsibilities as assigned that may include sending/receiving patient medical records; obtaining lab/X-ray reports, hospital notes, referral information, etc.; completing forms/requisitions as needed; scheduling appointments; verifying insurance coverage and patient demographics; managing and updating charts to ensure that information is complete and filed appropriately * Fulfills environmental responsibilities as assigned that may include setting up instruments and equipment according to department protocols; cleaning exam/procedure rooms, instruments, and equipment between patient visits to maintain infection control; cleaning sterilizer according to scheduled maintenance program and keeping appropriate records; ordering, sorting, storing supplies; and restocking exam/procedure rooms * Fulfills organizational responsibilities as assigned including respecting/promoting patient rights; responding appropriately to emergency codes; sharing problems relating to patients and/or staff with immediate supervisor * Fulfills clinical medical assisting responsibilities that vary according to state law, which may include medical/surgical asepsis, sterilization, instrument wrapping and autoclaving; checking vital signs or mensuration; physical examination preparations; clinical pharmacology; drug administration through various routes including injections; prescription verifications with physicians orders; minor surgery assists including surgical tray set-up pre/post-surgical care, applying dressings, and suture removal; biohazard waste disposal and monitoring; therapeutic modalities; instructing patients with assistive devices, body mechanics, and home care; laboratory procedures including Occupational Safety and Health Administration (OSHA) guidelines; quality control methods; CLIA-waived testing; capillary punctures and venipunctures; specimen handling such as urine, throat, vaginal, stool, and sputum; electrocardiography including mounting, emergency triage, and first aid.
