Essential Functions:

  • Ensures quality and safe delivery of health services to residents

  • Implements plan of care formulated by physician and DRC

  • Provides accurate and timely documentation of care given diagnosis or plan of care

  • Administers medication per physician orde Documents properly on MAR. Observes for any adverse reaction and notifies physician accordingly

  • Assembles resident charts, resident records and checks that all necessary forms are complete and accurate

  • Prepares incident/accident reports for residents as needed per policy. Notifies physician and family post incident/accident per policy and documents appropriately

  • Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with clinical standards

  • Assists resident in learning appropriate self-care activities

  • Organizes and monitors associates and material resources necessary for meeting the needs of the residents under his/her care

  • Recognizes emergencies and life-threatening situations and initiates appropriate action(s)

  • Maintains confidentiality and resident rights regarding all resident personal information

  • Performs any other duties requested by supervisor

Non-Essential Functions:

  • Leads by example exhibiting the CORE Values through servant leadership

  • Encourages teamwork and promotes company philosophy

  • Attends required community meetings and participates Life Enrichment activities when able

  • Completes all required courses in adherence with HRA University

  • Is prompt and able to perform the required duties of the position on a regular, predictable basis

  • Becomes familiar and understands how to report a missing person using the Safe Return program or equivalent

  • All associates are responsible for maintaining a safe and secure environment for all community residents


  • Graduated from an accredited program for LPN/LVN and is currently registered in the state of the community

  • Must obtain valid Continuing Education credits as required by the state of the community

  • Training in Alzheimer and Dementia with focus on Validation Therapy

  • Minimum of one (1) year nursing experience in a health care facility.

  • Experience in a long term care community and/or geriatric nursing facility preferred

  • Experience in emergency care and first-aid

  • Able to communicate effectively with all levels of management, associates, residents, family members, and outside contacts

  • Experience with Microsoft Office and Outlook software preferred

  • Able to make independent decisions

  • Able to seek out new methods and willing to incorporate them into existing practices when applicable

  • Must possess a passion to work with and around senior citizens

  • Must possess a passion to work with and around senior citizens

Ring up all sales on cash register properly and accurately, handling money, checks, and other types of payment received for products sold. Performs multi-function operation of fuel console, lottery machine, money order machine, telecom transactions, etc. Performs multi-function and cleaning duties necessary to maintain store cleanliness inside and out; basic upkeep and cleaning of all equipment at store. Inspect store facilities and equipment for safety, cleanliness, and proper working order. Contacts maintenance for repair when needed. Completes build-to's for ordering/purchasing merchandise. Receives and verifies vendor deliveries. Controls merchandise, cash shortages, and other selling expenses. Assists in maintaining proper inventory levels and shift audits. Assists new applicants with application process. Performs all duties with minimal supervision. Attends job-related meetings (may be required to work irregular hours). Performs other duties as assigned by the Store Manager.Working Conditions Perform approximately 95% of all work indoors, but will be required to work outside to clean parking lots, gas pumps, take out garbage, etc. Exposure to extreme cold temperatures while performing occasional work in a walk in cooler and/or freezer. Exposure to occasional noise. Work with a minimum direction and periodic supervision.Physical Functions Ability to stand and/or walk for up to 8 hours. Ability to occasionally lift and/or carry up to 30 pounds from ground to overhead up to 30 minutes of workday (i.e., assisting in stocking/maintaining inventory levels). Ability to occasionally lift and/or carry up to 60 pounds from ground to waist level (i.e., to replenish fountain syrups). Ability to push/pull with arms up to a force of 20 pounds (i.e., utilizing a hand-truck). Ability to bend at waist with some twisting up to one hour of workday. Ability to grasp, reach and manipulate objects with hands up to all day.
