Job Title: Licensed Practical Nurse
The LPN functions as a member of the patient care team under the supervision of the physician and/or Registered Nurse and assists in the delivery of patient care by data collection, observation, implementation and evaluation. The LPN will render nursing care in terms on individualized patient needs in conformance with recognized nursing techniques, procedures and established standards based on the scope of practical nursing.
Essential Duties and Responsibilities

Performs patient assessment and care planning, considering the patient's age group, severity of condition/disease to include physical, psychosocial, cultural, environmental, self care and educational needs
Utilize the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of the patient's care
Assess the patient; collect and document the patient's current health status and medical history
Teach patient/significant other appropriate health information in a timely manner and share written informational material
Communicate with the physician regarding patient needs, the nursing assessments, and recommendations on a daily basis, and as needed
Identifies patient/family individualized learning needs and implements measures to meet those needs based on patient/family capabilities to learn
Perform direct patient care, utilizing established procedures, policies, and standards
Demonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate books and manuals
Demonstrate ability to handle emergency situations in a prompt, precise, and professional manner
Schedules tests as directed by provider
Assists physician and office procedures and testing
Implements care in a safe, knowledgeable, skillful, consistent and continuous manner
Evaluates the effectiveness of equipment and processes for achieving positive patient outcomes
Adheres to the safety policies and procedures of SRPS
Recognizes and reports potential safety issues
Demonstrates ability to utilize biomedical equipment
Must be trained and competent in the Scheduling, MA/Nurse 100 & 200 functionality of CarePATH
Maintains confidentiality and actively supports patient/family rights
Supports and contributes to organizational cost-saving initiatives
Serves as a patient advocate
Administer medications according to practice policies and procedures
Maintain a clean, neat, and safe environment for patient and staff
Attends staff meetings as directed by the practice manager/supervisor
Assist patients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner
Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated, triages phone calls as necessary
Demonstrate sound judgment by taking appropriate actions regarding questionable findings or concerns
Ensure compliance with regulatory standards
Consistently demonstrate ability to respond to changing situations in a flexible manner in order to meet current needs, such as reprioritizing work as necessary
Minimize non-productive time and fill slow periods with activities that will enable you to prepare to meet the future needs of SRPS (education, organizing, housekeeping, assisting others)
Maintain regular attendance and do not incur excessive overtime
Performs all other duties as assigned
Demonstrates appropriate delegation of tasks and duties in the direction and coordination of healthcare team members, patient care and unit activities under the direction of the Practice Supervisor/Manager
Responsible for the work and training for a group of employees which may require work assignments, scheduling and performance improvement in creating team dynamics under the direction of the Practice Supervisor/Manager
Demonstrates appropriate conflict resolution skills
Awareness of and compliance with the Corporate Responsibility Program, which includes executing duties in accordance with all policies and regulations affecting one's job; and reporting all concerns or violations of policies, procedures or government regulations.
1 to 3 years experience preferred. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Certificates, Licenses, Registrations
Must have current, valid, unrestricted LPN license in the state of Ohio.
May be required to hold medication and IV privileges on state of Ohio LPN license
Education and/or Experience
Years of Experience:
1 to 3 years experience in physician office setting preferred.
Must be a graduate of an accredited school of nursing and hold a current, valid, unrestricted license in the state of Ohio.
Demonstrates assertive communication skills. Develops and maintains effective communication with internal and external individuals.
Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations. Ability to write reports and general correspondence. Employee must be able to explain instructions to patients and their families, and/or explain practice procedures to employees or visitors.
The employee is required to be able to communicate clearly. Some patient education is required.
Communication of instructions to patients and families upon check-out
The work requires the ability to analyze and solve problems that require the use of basic and/or advanced administrative/clinical experience based knowledge
Examples include performing multi-step procedure/tests, recognizing changes in patients' conditions, and analyzing priorities to facilitate optimum work performance
Ability to solve practical problems and deal with a variety of complex situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Seeks input/resources to facilitate decision making. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Failure to perform duties will result in serious customer consequences that may have short term or long term effects.
Ability to read and interpret physician orders, recognizes problem and resources available to facilitate decision making
Other Skills and Abilities
Works within a windows environment and is familiar with the functionality of the windows tools.
Microsoft Office Suite
Has proficiency working with the applications found within Microsoft Office Suite, including Microsoft Word, Excel, PowerPoint and Access.
Internet/Intranet: Has the ability to access the organization's intranet and is familiar with the different content and page layouts.
Can access information from the different sites within the web page. Can access the internet and search for relevant material as it relates to the current job.
Clinical Applications: PowerChart
Must be able to access the appropriate applications. Must be able to use the functionality of the applications for the successful completion of the employee's essential job requirements. Maintains the security of the system and access codes per hospital policy.
Must be trained and competent in the Scheduling and MA/Nurse 100 & 200 functionality of CarePATH and must be able to navigate in AS400, if applicable
