Recognized as a valuable member of the Rehabilitation team, the LPN practices under the supervision of the Rehabilitation Registered Nurse and assists patients in adapting to an altered lifestyle, while providing a therapeutic environment for patients and their families. Actions are team oriented to successfully provided patient/family care and high level of satisfaction, through evidence- based practice. Maintains a practice environment that reflects the ANA Code for Nurses, Philosophy of Nursing at UnityPoint Health within the structure of the Professional Nurse Practice Model and in collaboration with other care givers.

Responsibilities

Clinical Excellence & Patient Care

  • Possesses the knowledge and clinical skills necessary to provide care for people with physical disability and chronic illness

  • Participates in patient/family educational activities and uses appropriate resources to implement an individualized teaching and discharge plan with patients and their families.

  • Performs hands-on nursing care by utilizing the nursing process to achieve quality outcomes for patients.

  • Reinforces the care provided by other team members.

  • Provides direction and supervision of unlicensed assistive personnel (i.e. Patient Care Technician), demonstrates clinical judgment appropriate for Iowa's LPN scope of practice, uses problem solving techniques and time-management principles, and delegates appropriately.

  • Coordinates nursing care activities in collaboration with other members of the interdisciplinary rehabilitation team to facilitate achievement of overall goals.

  • Demonstrates effective oral and written communication skills to develop a rapport with patients, their families, and health team members and to ensure the fulfilment of requirements for legal documentation and reimbursement.

  • Maintains a strong commitment to the rehabilitation team.

  • Contributes to the assessment of the physical, psychological, sociocultural, and spiritual dimensions of patients and their families, as well as their educational and discharge needs.

  • Collaborates with other rehabilitation team members and contributes to the dynamic interdisciplinary plan of care.

  • Implements the interdisciplinary plan of care within the scope of LPN/LVN practice and provides nursing care and education directly or through ancillary personnel, as needed, to maintain and restore function and prevent complications and further lossontributes to the evaluation of the nursing care and to the modification of the plan as needed to achieve measurable goals and objectives.

  • Supports the goals for the rehabilitation patient developed by the rehabilitation team through collaboration with patients, their families, the rehabilitation staff nurse, and other disciplines that are oriented to wellness behavior and are reality based; and that encourage socialization with others.

  • Contributes to the interdisciplinary team process at team conferences and other team meetings and offers input into team decision making within their scope of practice.

  • Intervenes with team members and other healthcare professionals to ensure that the optimal opportunity for recovery is made available to the patient, the most significant member of the rehabilitation team.

  • Collaborates with the rehabilitation staff nurse and other team members to achieve cost-effective care by utilizing appropriate clinical measures to meet emergent physical, psychosocial, and spiritual situations.

  • Communicates effectively and interacts in a manner that facilitates the group process and team building.

  • Advocates and supports the rights of patients and families with care decisions.

  • Practice reflects knowledge of federal/state regulatory and accrediting agencies, i.e. Commission on the Accreditation of Rehabilitation Facilities (CARF) and the Iowa Department of Inspection and Appeals (DIA).

Clinical Excellence & Patient Education

  • Reinforces information provided to patients about the disease processes underlying disabilities and teaches nursing techniques to help patients and their families develop the self-care skills necessary to move toward wellness on the illness-wellness continuum.

  • Provides assistance to patients and their families within his/her scope of practice for future self-management and decision-making responsibilities by fostering patients' independence and goal achievement.

  • Reinforces the teaching done by the rehabilitation staff nurse and other specialists in rehabilitation and other healthcare disciplines and provides resource materials for patients' changing needs.

  • Demonstrates knowledge that patient and workplace safety is a priority by preventing errors, accidents and infections and reporting potential or actual concerns immediately.

Qualifications

Education:

Graduate of accredited nursing program.

Experience:

Experience in skilled nursing, long term care or acute rehabilitation.

License(s)/Certification(s):

Current licensure in good standing to practice as a Licensed Practical Nurse in Iowa.

Must possess and maintain current Basic Life Support (BLS) certification or obtain certification within three (3) months from date of hire.

Must obtain and maintain NIHSS education prior to end of orientation and maintain bi-annually

Completion of eight hours of stroke education prior to the end of orientation and annually

Valid driver's license when driving any vehicle for work-related reasons.

Knowledge/Skills/Abilities:

Writes, reads, comprehends and speaks fluent English.

Multicultural sensitivity.

Microsoft Office - basic computer skills.

Customer/patient focused.

Critical thinking skills using independent judgment in making decisions.

Other:

Use of usual and customary equipment used to perform essential functions of the position.

Work may occasionally require travel to other UPH facilities. May drive a UPH vehicle, rental or own vehicle.
