Reporting to the Coordinator of Care Nurse, you will be responsible for seeing patients in their home for revisits based on a skilled need You will implement the plan of care and evaluate the treatment You will document the visit electronically and completing documentation within 24-48 hours ursing, LPN, homecare, home care licensed practical nurse Qualifications: NY State LPN (Licensed Practical Nurse) License required Minimum of one year Med/surg experience in a hospital or Longterm Care facility required Drivers License and Car required Bilingual Spanish, Mandarin or Cantonese preferred
