The Licensed Practical Nurse (LPN) provides for and assists in meeting the TCU transitional care unit residents' physical, emotional and psycho-social needs while adhering to facility, state and federal guidelines for beautiful and vibrant skilled nursing care center in Wyoming, MN.

Nursing schedule includes;

• On call / casual

• Day, evening, over night shift; 12 hour shift

• Must pick up three shifts a month; one being a weekend shift

Meadows on Fairview TCU is located in the city of Wyoming, MN, steps away from the Fairview Lakes Hospital and a short distance from Forest Lake and North Branch.  We offer a home like atmosphere where you feel like you belong the minute you arrive!  Our 14-bed Transitional Care Unit is small enough to get to know your co-workers and residents yet large enough to provide you with opportunities and room to grow.

Responsibilities/Job Description

• Provides professional nursing cares and treatments

• Monitors work and performance of unlicensed staff

• Maintains current observation and documentation of resident health status

Qualifications

Required:

• MN LPN licensure

• AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers

• BLS is required upon hire and/or new hire must show proof that they are currently enrolled in a BLS class to renew or obtain their certification. BLS certification must be obtained within three-weeks of hire date and before nurse can perform independent practice

• 0-1 years of experience

• Ability to use technology in the form of computers, hand held devices, cell phones, and fax machines

Preferred:

• Vocational/Technical Training or Associate Degree

• 1-3 years of experience

• MS Office, Excel, and Word

• Point Click Care knowledge
