  • Directs and supervises the daily work assignments of the Resident Care Assistants

  • Is the first point of contact for the care associates regarding assignment questions or concerns

  • Participates in the hiring and termination processes as they relate to resident care associates

  • Has direct input into performance reviews for care associates and presents reviews with the RCD

  • Closely mentors, guides and directs the Lead Resident Care Assistants in all aspects of their job.

  • Coordinates care needs with community providers via an effective case management process

  • Measures and accurately records vital signs (temperatures, pulse, respiration, blood pressure weight and height) where appropriate as per state regulations

  • Accurately assesses, communicates, and documents residents' status

  • Observes and reports any significant changes in resident behavior and health to the DON, physician, family, RCA's, and Executive Director per state regulations

  • Successfully implements the plan of care

Job Requirements


  • Graduate of an approved LPN / Licensed Practical Nurse program (per state requirements)

  • Current state licensed as an LPN / Licensed Practical Nurse (as required by state regulation) and CPR certification

  • Previous experience working with the elderly in a paid or volunteer position highly desirable

VIEW JOBS 7/28/2021 12:00:00 AM 2021-10-26T00:00 ESU DSWII This position is at the Hogan Regional Center in Danvers on the Evaluation and Stabilization unit The Department of Developmental Services (DDS) seeks a caring and compassionate applicant for the role of DEVELOPMENTAL SERVICES WORKER II (DSW II) who is committed to building meaningful relationships in order to provide outstanding care and service. The DSW II provides direct care services and aid to individuals with intellectual and developmental disabilities by providing for the participants' needs in daily living and those skills necessary for community life. This position is for the Evaluation and Stabilization Unit at 1HC. This unit supports individuals in crisis many with violent and extreme behaviors. Duties include: Providing direct care services to individuals with intellectual disabilities many with extreme and violent behaviors; Implementing of ADL/ILS, dinning programs, implementing Coordinated Treatment Plans, Direct care staff implement behavioral and recreational programs; perform crisis intervention and ability to respond to severe and challenging behaviors; maintain confidentiality of individuals; accompany individuals to various appointments; provide leisure activities to individuals; assist individuals in meeting his/her personal care and nutritional needs; assist and support individual in making choices in his/her life; maintain individual's rights by complying with applicable laws and regulations; may drive individuals to work, appointments, and recreational outings; and perform related work as required. Duties and Responsibilities (these duties are a general summary and not all inclusive): * Assist in the implementation of habilitative, behavioral and recreational programs; perform program-related cooking and housekeeping duties; dispense medications in accordance with applicable laws, rules, and regulations. * Communicate about, and with, the individuals served in a manner which promotes their growth, human rights and integration into the community. * Assist, instruct and support individuals in understanding and exercising their rights to their fullest potential. * Report, as mandatory reporters, all abuse and neglect to the residential supervisor and to the Director of Residential support all incidences of abuse in accordance with regulations; and complete required paperwork. * May drive individuals to work, appointments and recreational outings. * Perform crisis intervention; and maintain confidential records of individuals * Provides for the physical welfare of clients by maintaining a safe and clean environment; notifying nursing staff of Health related issues and performs other related assignments for the purpose of providing optimal care to the Individuals of Hogan. * Participates in the development and implementation of Individual Service Plans by carrying out, monitoring and Documenting activities of daily living, behavior, developmental training, leisure and other related programs and assignments to aid the individuals in developing to their full potential * Maintains individual's rights by complying with Department of Developmental Services: Title XIX and Facility Regulations for the purpose of recognizing the rights of the individual. * Adheres to Facility and Unit Policies and Procedures by complying with said policies for the purpose of delivering quality services to Hogan clients and their families. * Assists in providing supports to individuals on behavioral unit who exhibit violent and extreme behaviors. * Assists in fostering familial relationships i.e., by assisting residents with phone calls and monitoring for appropriate interaction; Provides direct care services when needed to individuals with violent and extreme behaviors; Acts as role model to other staff in implementing all supports; Interfaces with clinical team; may be lead staff in implementation of behavioral interventions such as physical restraint and/or mechanical restraint Provide supports to individuals on behavioral unit who exhibit violent and extreme behaviors. * Must successfully pass PABC Preferred Qualifications: * Knowledge of the comprehensive service needs of persons with developmental/intellectual disabilities and their families/guardians. * Ability to give written or verbal instructions in a clear and understandable manner, and to plan, assign and coordinate the activities of others. Agency Requirements: * Must possess current and valid Class D Motor Vehicle Operator's License; * Applicants must pass the Medication Administration Program (MAP) certification; * After successfully completing a training program, the selected candidate must acquire and maintain; Cardiopulmonary Resuscitation (CPR) and First Aid (FA) certification. Please Note: * This position is designated as an "EMERGENCY" position in the Department of Developmental Services Emergency Plan. This means that whenever the Governor declares that the Commonwealth is in a state of emergency, selected applicant will be required to report to work. This is a 24hour/7 days a week staff position. Candidates must be able to be flexible in their work schedule. The Department of Developmental Services requires employees in this position to be able to perform "MANDATORY OVERTIME" when circumstances dictate the need. * This position is located in Danvers, MA at the Hogan Regional Center's Evaluation and Stabilization Unit * This position is located at 1HC/ESU (Evaluation and Stabilization Unit).
