Shrewsbury Crossings, located in Shrewsbury MA, is seeking caring and compassionate LPN's or RN's to join the amazing team Part Time 7AM-2PM, 9AM-2PM, 2PM-9PM and 5PM-9PM opportunities. We are offering a $1,500 employment bonus! (*bonus for FT and PT (Per Diem status excluded) is payable in 2 increments- $750 at 90 days and $750 at 120 days- individual must be gainfully employed with Benchmark at the time of payout)

Responsibilities for the LPN/RN include:

Direct Supervision

  • Directs and supervises the daily work assignments of the Resident Care Assistants

  • Is the first point of contact for the care associates regarding assignment questions or concerns

  • Participates in the hiring and termination processes as they relate to resident care associates

  • Has direct input into performance reviews for care associates and presents reviews with the Resident Care Director (RCD)

  • Has direct input into disciplinary documentation for care associates and presents action with the Resident Care Director (RCD)

  • Closely mentors, guides and directs the Lead Resident Care Assistants in all aspects of their job.

Resident Care

  • Coordinates care needs with community providers via an effective case management process

  • Measures and accurately records vital signs (temperatures, pulse, respiration, blood pressure weight and height) where appropriate as per state regulations

  • Effectively balances service demands with supporting resident independence

  • Accurately assesses, communicates, and documents residents' status

  • Observes and reports any significant changes in resident behavior and health to the DON, physician, family, RCA's, and Executive Director per state regulations

  • Successfully implements the plan of care

  • Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations

  • Responds to emergencies and personal emergency response system and knows when to call for backup

Job Requirements


  • Graduate of an approved LPN / Licensed Practical Nurse program (per state requirements) or RN/ Registered Nurse program (per state requirements)

  • Current state licensed as an LPN / Licensed Practical Nurse (as required by state regulation) or RN/ Registered Nurse program (per state requirements) and CPR certification

  • Previous experience working with the elderly in a paid or volunteer position highly desirable

Position: RN or LPN Hours: 8:30 - 5:30 pm - hours vary Department: Adult Medicine Phone Triage 85% - 90% Responsibilities Responsible for the effective delivery and management of direct patient care utilizing the nursing process and adhering to standards of nursing practice. Provides appropriate care to patients ranging within the ages of infants to geriatrics. Provides pre-visit and post-visit telephonic support and patient education, populating the Electronic Medical Record with historical information, lab findings, and preliminary orders to be reviewed by the physician at the next clinic visit. Ad hoc visits to the clinical sites will also be scheduled to enhance coordination of team-based care. Qualifications * Graduate of an accredited School of Nursing.
