Position Summary: Receives referrals from facilities and physicians and enters them into established software system. Acts as a resource for field clinicians and patients.

Obtains and writes verbal orders from the physician. Performs coding and QA audits. # # Position Qualifications: Minimum Education:

Nursing Diploma Minimum Experience: 1 year of med/surg or acute care experience in an institutional setting. Preferred Experience: 1 year of prior home care experience Required Certification/Registration: Current WV LPN licensure COS-C and HCS-D certification desired.

Exposed to unpleasant elements (accidents, injuries, and illness) Subject to varying and unpredictable situations Handles emergency or crisis situations Some Travel required
