The LPN Intake Coordinator functions as a liaison to referral sources by responding to agency referrals and facilitating services as quickly as possible. Manages all aspects of the patient intake process for both home health and hospice care. Remote flexible, however does require in person attendance for staff trainings and meetings. • Receives and requests physicians orders.

• Overseas the department to ensure the best possible patient care.

• Gathers information from referral sources and ensures eligibility for insurance coverage.

• Assists in assigning and scheduling clinicians

• Evaluates referrals for appropriateness.

WA state LPN license
