LPN Hospice

Thomas Talent Network, LLC is currently seeking a compassionate and dedicated LPN to join our hospice team. As an LPN Hospice, you will provide skilled nursing care and support to terminally ill patients in their homes or hospice facilities. You will work closely with a multidisciplinary team to ensure that patients receive high-quality palliative care and support during their end-of-life journey.

Responsibilities:

• Perform nursing assessments and develop care plans based on patient needs
• Administer medications and treatments as prescribed
• Assist with pain and symptom management to provide comfort to patients
• Educate patients and families on self-care techniques and end-of-life issues
• Provide emotional support to patients and families during difficult times
• Collaborate with other healthcare professionals to coordinate patient care
• Document patient care activities and communicate effectively with the care team

Requirements

Requirements:

• Valid LPN license in the state
• Minimum of 1 year of experience in a healthcare setting, preferably in hospice
• Strong clinical and critical thinking skills
• Compassionate and empathetic approach to patient care
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Flexible schedule, including availability for on-call shifts
• Valid driver's license and reliable transportation
