How You'll Help Transform Healthcare:

Roanoke Region. Full time. Mileage reimbursement, as well as company equipment and safety equipment.

Assists the RN in performing care for patients of all ages in the home specific to the individual patient needs as ordered by the physician and supervised by the registered nurse. Care must be directed toward specific goals and documentation must reflect care given as well as discharge planning.

• Provides quality skilled nursing care and treatments to patients of all ages as

• assigned;

• Understands and respects the Home Health/Hospice philosophy of care:

• Assists the RN care manager in coordinating all aspects of patient care;

• Follows the interdisciplinary team's/RN's direction in coordinating delegated aspects of car;

• Responsible for completing and submitting documentation according to

• agency timeliness standards;

• Responsible for abiding by all established hospital/departmental rules and

• regulations;

• Responsible for abiding by all established office procedures;

• Participates in continuing education and other meetings and;

• Promotes Carilion Culture of Excellence

• Demonstrates professionalism and enthusiasm in daily activities

What We Require:

Education: Graduate of approved school of Practical/Vocational Nursing

Experience: Must have at least one (1) year of experience in med-surg nursing. Home health experience preferred.

Licensure, certification, and/or registration: Licensed Practical Nurse in the Commonwealth of Virginia. Must possess valid Virginia driver's license.

Other Minimum Qualifications: Must have own form of reliable transportation and clear criminal record check.
