• PRN Job Locations US-PA-CAMP HILL | US-PA-YORK ID 2024-149462 Line of Business Adoration Home Health and Hospice Position Type PRN Our Company Adoration Home Health and Hospice Overview Grane Home Health is seeking a PRN LPN - Licensed Practical Nurse to join our team. The Home Health LPN will make intermittent home-vistis to our skilled nursing patients in a particular area.

Our comprehensive benefits include: Flexible schedule Competitive pay mileage reimbursement 401(k) with Company match Tuition reimbursement other discounts Responsibilities Communicates health/nursing needs and/or recommendations to RN supervisor Coordinates admissions, discharges, and follow-up as needed Assists in medical/health appointments as needed Ensures the provision of nursing/health services under the direction of the RN and as ordered by the physician Provides direct care as outlined in the Service Plan/Nursing Care Plan Takes and records vital signs, weights, wound measurements, etc.
