Responsibilities

Completes functions related to the intake process for Home Health patients for assigned team including workflow within the Electronic Medical Records (EMR) system. Responsibilities include, but are not limited to, managing the intake process for Home Health, setting up patient account information, and facilitating face-to-face physician encounter visit.

Reviews, approves and coordinates all referrals directed toward assigned care team. Manages all assigned intake/admission EMR workflow for Home Health patients (initial referrals, physician verification, etc.) per assigned care team. Provides daily reporting to care team regarding pending admissions, new admission and outstanding (open) items. Obtains initial payer source eligibility verification upon intake/referral and coordinates indicated follow-up necessary to adequately communicate the patient's expected out-of-pocket expense for services. Coordinates with care team in facilitating completion of physician face-to-face patient appointment/encounter visit. Responsible for the face-to-face encounter workflow process as assigned within the EMR, which includes contacting the physician's office to verify visit or schedule visit, request visit documentation, and log activity within the EMR. Conducts or assists with special projects as requested by the Administrator and other Brookdale managers. Builds and maintains positive relationships and rapport with co-workers, associates, patients and their family members and community personnel. Adheres to communication guidelines established by Brookdale and local and regional management. Provides professional customer service to associates, patients, physicians, and other referral sources. Maintains a safe and clean work area and adheres to facility/corporate safety standards. Adheres to established confidentiality standards as required by Brookdale and HIPAA concerning community and patient information. Completes special projects as assigned by the management team.

Qualifications

Education and ExperienceAssociate/Bachelor degree in nursing and current Licensed Practical Nurse (LPN) license required. A minimum of two years of experience in an office setting, at least one year of home health agency experience preferred. Proficiency with computer use and software applications including MS Office software products required and experience with EMR software preferred.

Certifications, Licenses, and Other Special RequirementsLicensure as a Practical Nurse in the respective state required. Valid driver's license and proof of automobile liability insurance.

Management/Decision MakingUses limited independent judgment to make decisions based on precedents and established guidelines. Solves problems using standard procedures and precedents. Knows when to refer issues to supervisor and when to handle them personally.

Knowledge and SkillsHas a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has a working knowledge of the organization. Must be able to work under stress and in a fast-paced environment. Demonstrate professional conduct and ethics according to agency policy and procedure. Demonstrate compliance with agency policies and procedures and federal, state, and local regulations. Must maintain confidentially of patient information and all agency operations.

Performs duties as directed by Case Manager or Clinical Team Manager. Essential Duties & Responsibilities: * Assist the Registered Nurse or Physician in performing specialized procedures and duties. * Provides prescribed nursing care procedures to assigned clients. * Participates in assessing, planning, implementing and evaluating client care in collaboration with an RN. * Provides health teaching and assist the clients and families in learning self-care techniques. * Communicates and documents findings in a factual, concise and timely manner per agency policy. * Documents services, client condition and response to treatment. * Demonstrates safety precautions in compliance with OSHA, CHAP, and Federal and State standards. * Prepares clinical and progress notes. * Operates medical equipment correctly for client care. * Demonstrates knowledge and skills necessary to provide appropriate client care. * Obtains medical orders which are carried out promptly. * Recognizes and communicates changes and potential client problems in a timely manner, utilizing the proper chain of command. * Uses good hand washing and aseptic techniques. * Performs client care duties proficiently and safely; administering medications, performing procedures and treatments per agency policy. * Demonstrates knowledge of the principles of growth and development over the life span. * Participates in the PI program by utilizing suggestion boxes, customer service, and brings matters to attention of management staff. * Updates management regarding potential problems or concerns.
