Your responsibilities:

  • Provide direct care to residents, while promoting the highest level of functioning, as directed by management

  • Monitor residents as needed, including resident responses to treatment and care plans

  • Supervise and direct nursing assistants as needed

  • Promote quality services within community standards, state and federal regulations

  • Document necessary information regarding nursing care, care plans and observation of residents' overall conditions

Required skills and qualifications:

  • Current practical nursing licensure within the state of practice

  • Current CPR and first aid certifications a plus

  • One year of relevant nursing experience

  • Flexible schedule, including availability to work evenings, weekends and holidays as needed

  • Must enjoy working with the senior population

The Home Health Aide is supervised by a registered nurse or therapist every two weeks, either when the Home Health Aide is present to observe and assist, or when the aide is absent, to assess relationships and determine whether goals are being met. PSD supervision by RN occurs every 30 days. The position shift can be between 20-40 hours per week. Job Duties: Helps clients with dressing and undressing and maintaining proper clothing. Assists clients in maintaining proper and clean appearance. Assists clients with bathing and maintaining cleanliness of all body areas. Helps clients with toileting needs. Helps clients with meals and in meeting dietary needs. Assists clients in transferring, repositioning and walking. Reminds clients of self-medication as per care plan, reports deviations. Respects clients' property and assists clients to safeguard personal items. Assists clients to keep personal space clean and orderly. Maintains clients' well-being, privacy and confidentiality of personal information. Complies with HIPAA guidelines. Provides clients with opportunities for independence and choice as appropriate. Accepts additional responsibilities as deemed appropriate by the supervisor. Qualifications High school education or higher education is not required to be hired into this job. At least six months prior work experience in a health care facility preferred. Completion of an accredited nursing assistant / HHA training program with written skills test and competency evaluation, and formal certificate obtained. Must be at least 18 years of age.
