Providence is calling LPN (1.0 FTE, Days) to Providence Cedar Hills Home Services in Portland, OR.

In this position, you will:

  • Provide basic patient care for patients in their place of residence and specific delegated duties that support the care of the Home Health patient and their family as directed by the registered nurse (RN) case manager or the clinical supervisor.

  • Perform all duties in compliance with all applicable State and Federal regulations and in compliance with all regulatory bodies.

Required qualifications for this position include:

  • Graduation from an accredited Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) school of nursing

  • Licensed or licensure eligible to practice in the State of Oregon

  • Current American Heart Association Basic Life Support (AHA BLS) for Health Care Provider within 30 days of hire

  • Valid Oregon driver's license, reliable transportation, and automobile liability insurance

  • 2 years of experience (Acute or Long Term Care (LTC) care)

VIEW JOBS 3/13/2020 12:00:00 AM 2020-06-11T00:00 Providence is calling a Physical Therapy Assistant - Home Health West Portland (1.0 FTE, Days) to Cedar Hills Home Services in Portland, OR We are seeking a Physical Therapy Assistant that will provide treatment of neurological, orthopedic, and muscular disorders in accordance with treatment plan established by Physical Therapist to provide efficient, effective physical therapy to referred geriatric participants. Utilizes artificial aids such as prosthetic devices, therapeutic exercise techniques, emotional support and educational training to restore mobility and function to participants, reduce pain, and promote healing. Provide home care coordination. In this position you will have the following responsibilities: * Provide care appropriate to the age of the geriatric participants enrolled in the program * Provide for efficient, effective physical therapy by administering the treatment program established by the physical therapist. The physical therapy assistant positions the geriatric participant, sets up equipment and administers treatments as indicated in physical therapy treatment plans * Contribute to an effective, efficient treatment program by observing, recording and reporting geriatric participant treatment sessions * Contribute to the effective, efficient provision of treatment by assisting other physical therapy staff * Contribute to an orderly service by maintaining treatment areas. This includes organizing equipment prior to a treatment; cleaning equipment after use; changing linen; removing laundry; and requesting repair or replacement of broken equipment Required qualifications for this position includes: * Graduate of a Physical Therapy Assistant program approved by the American Physical Therapist Association * Licensed in the State of Oregon as a Physical Therapy Assistant * BLS (Healthcare Provider) * One (1) year experience in working with the frail or elderly population * Valid driver's license and reliable transportation * Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire Preferred qualifications for this position includes: * One (1) year experience in the area of inpatient/SNF working with Geriatric population and medically complex patients.
