Devereux Advanced Behavioral Health Adult Services provides programs and services for adults with intellectual and developmental disabilities. Located in Berwyn PA, this position is responsible for identifing health care needs and med pass.

The LPN will have the following primary focuses in this full time position: 1) Care Coordination as part of a patient-centered medical home. The LPN will reach out to some of the practice's hospitalized patients. They will work with patients with chronic conditions who need extra assistance coordinating appointments, understanding their disease, and also simply following up with them as directed by the physician. Patient teaching will be involved as well (diabetes, Coumadin tracking, etc.) 2) Triage: The LPN will help triage the patients' needs/symptoms under physician guidance. 3) Act as the lead clinical point person for Patient Centered Medical Home (PCMH). The LPN will be responsible for leading the clinical PCMH initiatives. They will be responsible for implementing new PCMH initiatives developed by the Best Practice Work group, see that they are maintained in the office, and be a support for our Medical Assistant staff as they adopt the new initiatives.
