The LTSS Specialist must have excellent communication skills, be highly motivated, pay attention to detail and able to work independently. The LTSS Specialist is responsible for assessing, determining eligibility based off LTSS program criteria and researching providers for service delivery of a wide range of services and supports. The goal of LTSS is to find services and supports for eligible elderly or disabled individuals allowing them the opportunity to be able to continue to live at home or in the community of their choice. In addition to working directly with individuals or consumers, the LTSS Specialist also provides information and referral services to the public and family members about aging and community resources. This position is responsible for assessing and determining eligibility for a wide range of services that would benefit individuals at risk of having to leave their own homes and providing information and referral services to the public and family members about aging and community resources
This position may be required to work a flexible schedule which may include evening, night, holiday and weekend hours. Travel may be required in a multi-county area.
The Ideal Candidate Will Have:
A bachelor's degree with major work in social work, psychology, health or related field may be considered as applicable to the entry-level knowledge, skills and abilities.
