Westminster-Canterbury on Chesapeake Bay is hiring a Full Time Long-Term Care Unit Manager RN for The Hoy Center.

The Hoy Center encompasses Westminster Canterbury's long-term care (LTC), rehab, and pre-hab services.

What will I do?

This is an exciting Nurse leader position that is primarily responsible to manage the overall care for the residents on the long-term care units within The Hoy Center. This will include providing clinical support, guidance, and oversight for the delivery of resident-centered nursing care in the Hoy Healthcare Center and specifically for our memory care unit. Although you will not be responsible for direct reports, you will demonstrate and promote a culture of respect, passion, and professionalism.

You will use your Clinical expertise and knowledge to effectively delegate and assign clinical responsibilities and provide oversight in the delivery of quality and consistent care. Your leadership will be focused to ensuring a strong and survey-ready team, providing the best care to residents and the best support to one another.

You will also demonstrate compliance and promote the same with all team members. You will serve as a member of the Quality Assurance and Performance Improvement Committee (QAPI) and interdisciplinary team meetings. You will be responsible for the oversight and execution of plan of care interventions responsive to risk-related events such as falls, skin conditions, and other clinical care areas.

Specifically, you will lead the memory care unit to ensure person-centered care is being delivered, team members are engaging with residents within the unit in positive ways, and resident preferences and rights are being honored. The environment in the memory care unit should reflect our intentional approaches in working with residents with dementia, using the Montessori method, Pathways to Engagement, and other models of care delivery. You will partner with Therapeutic Recreation to best serve residents' needs.

You will be working community-wide with leaders to identify, address, and resolve the concerns that may impede us meeting our goals. You will represent WCCB with regulatory agencies and representatives, as well as with the family members and guests of residents.

You will be the nursing representative at long-term care interdisciplinary care plan meetings. This means you will update participants on nursing-related focus areas, problem-solve, and update the care plan to meet the residents' goals/ preferences as needed, then transfer necessary actionable items to the direct care team. You may at times assist MDS Coordinators with resident interviews or other tasks as needed.

What do I need for this role?

  • Current, unencumbered Registered Nurse (RN) license in the State of VA (or compact license)

  • Current CPR Certification

  • Physically able to perform all aspects of this job

  • This position requires someone who is highly motivated, energetic, and able to work both independently and under the direction WCCB leadership.

  • Must be a multi-tasker, with the ability to manage pressure and adjustments tactfully, and with ease.
